老年人糖脂代谢病的中医证候要素调查

Investigation of Traditional Chinese Medicine Syndrome Elements of Glucolipid Metabolic Disorders in the Elderly

目的 探讨老年人糖脂代谢病的中医病位和病性证候要素分布规律。方法 收集60岁及以上的糖脂代谢病患者3459例,对证候要素进行频数分析和聚类分析。结果 3459例糖脂代谢病老年人群的病位证素以肾、肝、脾多见。病性证素分布前5位依次为:血虚、痰、阴虚、热、湿。糖脂代谢病中医证候要素组合规律多样,病位证素2种以上的组合形式占比21.8%(753/3459),病性证候要素2种以上的组合形式占比64.0%(2215/3459)。聚类分析发现4种证候要素组合规律:气滞血瘀、肝阳上亢、脾阳气虚;痰湿证;热证;肾阴虚证与血虚证。结论 老年人群糖脂代谢病的常见病位证素是肾,涉及肝、脾、心和胃,常见的病性证素是血虚、痰、阴虚、热、湿等。

Objective To explore the distribution characteristics of traditional Chinese medicine(TCM)disease location and pathogenic syndrome elements of glucolipid metabolic disorders(GLMD)in the elderly.Methods This study collected 3459 patients with GLMD aged≥60 years,and the syndrome elements were analyzed by frequency and cluster analysis.Results The pathogenic syndromes elements of 3459 elderly patients with GLMD were mostly found in the kidney,liver and spleen.The top five distributions of pathogenic syndrome elements were blood deficiency syndrome,phlegm syndrome,yin deficiency syndrome,heat syndrome,and dampness syndrome.The combination pattern of TCM syndrome elements in GLMD was diverse.The combination form of more than 2 kinds of disease location syndrome elements accounted for 21.8%(753/3459),and the combination form of more than 2 kinds of disease syndrome elements accounted for64.0%(2215/3459).Cluster analysis found the combination law of four syndromes as follows:qi stagnation and blood stasis,liver-yang hyperactivity,spleen yang qi deficiency,phlegm-dampness syndrome,heat syndrome,kidney yin deficiency syndrome and blood deficiency syndrome.Conclusion The common disease location syndrome elements of the elderly people with GLMD are kidney,involving liver,spleen,heart,and stomach and common pathological syndrome elements are blood deficiency syndrome,phlegm syndrome,yin deficiency syndrome,heat syndrome,damp syndrome,etc.
