Experience of Professor Tian Yaozhou in Treating Chronic Atrophic Gastritis

慢性萎缩性胃炎(CAG)是消化系统常见病,具有病情反复、缠绵难愈的特点。病理主要以胃腺体的萎缩、伴或不伴有肠上皮化生或异型增生为特征。肠上皮化生和异型增生属于癌前病变,与胃癌的发生关系密切,积极控制CAG患者的临床症状,延缓其病理进展或逆转其病理变化,有利于改善患者的生活质量、减轻经济负担及预防胃癌发生。田耀洲教授认为此病属本虚标实,脾胃虚弱是本,气滞、湿郁、瘀血为标,虚、滞、湿、瘀相互胶结推动着疾病的进展,在辨证上推崇微观与宏观相结合,用药上推崇中西医结合,并擅用经方与药对,精准辨治与用药。田教授开创的自拟方剂芪芝方,疗效显著,可有效改善脾胃湿热型CAG的临床症状。今将田教授辨治CAG之经验及自拟方剂分享给大家,以期为临床辨治CAG提供参考借鉴。

Chronic atrophic gastritis is a common disease of digestive system,with long course of disease and difficult to cure.It is pathologically characterized by atrophy of gastric glands,often with or without intestinal metaplasia or dysplasia.Intestinal metaplasia and dysplasia are kind of precancerous lesions,which are closely related to the occurrence of gastric cancer.Therefore,active treatment of chronic atrophic gastritis,delaying or reversing its pathological changes,is of great significance for improving patients'quality of life and preventing the occurrence of gastric cancer.Professor Tian has studied traditional Chinese medicine(TCM)for more than40 years,with rich theoretical knowledge and practical experience in treating CAG with TCM.Professor Tian thinks that pathological properties of CAG is deficiency and excess syndrome.The root cause of CAG is spleen-stomach weakness and its pathological factors include qi stagnation,static blood and dampness turbidity.The combined effects of these pathological factors promote the development of CAG.Professor Tian advocates combining the micro and the macro in syndrome differentiation and treating CAG with integration of traditional Chinese and Western medicine.He specialized in ancient formulas and herb pairs.Qizhi formula created by Professor Tian has remarkable curative effect,which can effectively improve the clinical symptoms of CAG patients.
