凝血指标与血小板参数指导急性上消化道出血患者输血治疗的临床价值

The clinical value of blood coagulation and platelet parameters in the treatment of acute upper gastrointestinal hemorrhage

目的分析凝血指标与血小板参数指导急性上消化道出血患者输血治疗的临床价值。方法选取2015年2月至2020年2月鄂州市中医医院收治的120例急性上消化道出血患者作为实验组,另选取同期120名健康体检者作为对照组。比较实验组与对照组、实验组输血治疗前后凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIb)、凝血酶时间(TT)、血小板计数(PLT)、血小板平均体积(MPV)、血小板分布宽度(PDW)。结果实验组APTT、PT、TT均长于对照组,Fib水平、PLT均低于对照组,PDW宽于对照组,MPV小于对照组,差异有统计学意义(P<0.05)。治疗后,实验组APTT、PT、TT均短于治疗前,Fib水平、PLT均高于治疗前,MPV小于治疗前,PDW窄于治疗前,差异有统计学意义(P<0.05)。结论凝血功能和血小板相关参数可鉴别上消化道出血,APTT、Fib和PLT可为上消化道出血患者临床输血治疗提供参考价值,值得临床推广应用。

Objective To analyze the clinical value of blood coagulation and platelet parameters in the treatment of acute upper gastrointestinal hemorrhage.Methods 120 patients with acute upper gastrointestinal hemorrhage admitted to E'zhou Hospital of Traditional Chinese Medicine from February 2015 to February 2020 were selected as the experimental group,and 120 healthy subjects were selected as the control group.The coagula-tion time(APTT),prothrombin time(PT),fibrinogen(Fib),thrombin time(TT),platelet count(PLT),mean platelet volume(MPV)and platelet distri-bution width(PDW)were compared between the experimental group and the control group,the experimental group before and after blood transfu-sion.Results APTT,PT and TT in the experimental group were longer than those in the control group,Fib level and PLT were lower than those in the control group,PDW was wider than that in the control group,MPV was smaller than that in the control group,and the differences were statistical-ly significant(P<0.05).After treatment,APTT,PT and TT in the experimental group were shorter than those before treatment,Fib level and PLT were higher than those before treatment,MPV was smaller than that before treatment,PDW was narrower than that before treatment,and the differ-ences were statistically significant(P<0.05).Conclusion Coagulation function and platelet parameters can distinguish upper gastrointestinal hemor-rhage,APTT,FIB and PLT can provide reference value for clinical blood transfusion treatment of patients with upper gastrointestinal bleeding,which is worthy of clinical promotion and application.
