摘要
目的评价综合护理在晚期糖尿病肾病血液透析患者中的效果和价值。方法选取2019年9月至2023年9月就诊于南京市六合区人民医院的50例晚期糖尿病肾病患者,采用随机数分配方法分组,每组各25例。研究组采用包括透析优化、营养个体化指导、并发症精细管理、心理深度干预、康复训练等措施在内的全方位护理,对照组接受常规护理,比较两组患者血糖、营养状况以及生活质量情况。结果研究组患者的空腹血糖(Fasting Plasma Glucose,FBG)、餐后2小时血糖(2 Hour Postprandial Plasma Glucose,2hPG)以及糖化血红蛋白(Glycated Hemoglobin,HbA1c)水平相较于对照组明显下降(P<0.05);研究组患者的体质量指数和上臂围指标有所提升,优于对照组;研究组在营养风险筛查2002(Nutritional Risk Screening 2002,NRS2002)中的评分较对照组低(P<0.05);研究组在心理功能、社会功能、躯体功能、环境因素以及整体评价方面的得分均高于对照组(P<0.05)。结论综合护理明显改善了晚期糖尿病肾病血液透析患者的血糖控制、营养状态、生活质量和降低了并发症率。
Objective This study evaluates the effectiveness of holistic nursing care in end-stage diabetic nephropathy patients undergoing hemodialysis.Methods Fifty patients at Nanjing Liuhe District People's Hospital from Sept 2019 to Sept 2023 were randomly split into two groups of 25.The study group adopts comprehensive nursing measures including dialysis optimization,individualized nutrition guidance,precise management of complications,deep psychological intervention,rehabilitation training,etc.,while the control group received regular nursing.Comparisons were made regarding blood sugar,nutritional status,and quality of life between the two groups.Results The study group exhibited more significantly reduced fasting/postprandial glucose and HbA1c levels,increased BMI and arm circumference,and decreased nutritional risk than the control group.Quality of life,encompassing psychological,social,physical,and environmental aspects,along with overall and total scores,showed greater improvement in the study group than in the control group(P<0.05).Conclusion Holistic nursing markedly improves blood sugar management,nutrition,quality of life,and lowers complication rates in advanced diabetic nephropathy patients undergoing hemodialysis.
