中医非药物疗法治疗导尿管相关膀胱刺激征的研究进展

Research progress in non-pharmacological therapies in traditional Chinese medicine for catheter-related bladder discomfort

导尿管相关膀胱刺激征(catheter-related bladder discomfort,CRBD)是指患者因置入导尿管导致膀胱逼尿肌异常,从而导致膀胱过度活动的一组临床表现,表现为尿频、尿急和耻骨联合上区不适感等临床症状。中医认为,肾虚膀胱湿热,气化功能失调是膀胱刺激征的病因病机。中医非药物疗法在患者无法口服或因药物副作用过大不能耐受的情况下,与西医预防性治疗相比,显现出其临床有效、不良反应发生率较低、多为一过性、患者易接受、减少术中麻醉药物的使用量及降低麻醉药物副作用等独特的优势。但中医非药物疗法的相关研究仍缺乏更多的循证医学证据支持,故未来应着重开展该领域的临床研究及基础研究探索。

Catheter-related bladder discomfort(CRBD)refers to a group of clinical manifestations caused by bladder detrusor muscle abnormalities during catheter insertion,resulting in bladder overactivity.These clinical symptoms include frequent urination,urgency of urination and discomfort in superior pubic symphysis.Traditional Chinese medicine(TCM)believes that kidney deficiency,dampness heat of bladder,and dysregulation of qi in the human body are the etiology and pathogenesis of irritation symptoms of bladder.When patients are unable to take oral medicine or can not tolerate due to excessive side effects of drugs,TCM non-drug therapy shows its unique advantages such as clinical effectiveness,low incidence of adverse reactions,mostly transient,easy for patients to accept,reducing the dosage of intraoperative anesthetic drugs used,and reducing the side effects of anesthetic drugs compared with Western medicine preventive therapy.However,the related researches of TCM non-drug therapy still lack more evidence support of evidence-based medicine.Therefore,clinical research and basic research exploration in this field should be emphasized in the future.
