湖南省53家医疗机构伤口造口门诊开展现状的调查研究

Status quo of wound and stoma outpatient departments in 53 medical institutions in Hunan province

目的:了解湖南省医疗机构伤口造口门诊开展现状,为伤口造口门诊的管理及相关政策制订提供参考依据。方法:2022年11月—12月采用自行设计的伤口造口门诊开展现状调查问卷,对湖南省医疗机构伤口造口门诊的运行情况、设施配备及资源配置情况、出诊护士情况及门诊发展中的困难进行调查。结果:共53家医疗机构伤口造口门诊及127名出诊护士完成调查。其中,三级甲等医院36家(67.92%),三级乙等及以下医院17家(32.08%)。不同等级医疗机构伤口造口门诊在院外会诊、专科护士培训等运行情况,延续性护理平台建设等设施配备、亲水性敷料等资源配置方面的差异具有统计学意义(P<0.05)。三级甲等医院伤口造口门诊护士专科工作年限较长、省外进修经历较丰富且上一年度论文发表篇数较多(P<0.05)。50家(94.34%)医疗机构伤口造口门诊反映伤口造口专科护理价格偏低,48家(90.57%)医疗机构伤口造口门诊认为赋予专科护士处方权可促进门诊发展。结论:湖南省伤口造口门诊稳步发展,其在满足患者健康需求、护理专科人才培养等方面取得了一定成效,未来运行发展过程中应加大软、硬件设施投入,优化护理资源配置,完善相关支持政策,促进伤口造口门诊可持续发展。

Objective:To understand the current situation of wound and stoma outpatient departments in Hunan Province,and to provide reference for the management and the formulation of relevant policies.Methods:From November to December 2022,a self-designed questionnaire was used to investigate the operation status,facilities,resources,nursing staff and difficulties met in the development of these wound and stoma outpatient departments in 53 medical institutions in Hunan Province.Results:A total of 53 such outpatient departments and 127 staff nurses were surveyed.According to the survey,36(67.92%)outpatient departments belonged to tertiary Grade A hospitals and 17(32.08%)belonged to tertiary Grade B hospitals and below.Statistically significant differences were observed when comparing the operation status such as out-hospital consultation and specialist nurse training,facilities such as continuous nursing platform construction and resources such as hydrophilic dressing(P<0.05)among wound and stoma outpatient departments at different levels.Nurses working in tertiary Grade A hospitals had longer professional working years,richer learning experience outside the province,and more papers published(P<0.05).50(94.34%)departments complained about that the low price of wound and stoma care,and 48(90.57%)departments thought that granting the prescription right to specialist nurses could promote their development.Conclusion:The wound and stoma care departments in Hunan Province have developed steadily,which have achieved certain results in meeting the health needs of patients and training specialized nursing personnel.In the future,we should invest more in hardware and software facilities,optimize the allocation of nursing resources and improve relevant support policies,so as to promote the sustainable development of the wound and stoma care departments.
