CT增强及血管成像扫描检查中碘对比剂外渗发生的影响因素调查及干预对策

摘要 目的:分析CT增强及血管成像扫描检查中碘对比剂外渗发生的影响因素调查,并探讨干预对策。方法:选取2019年1月—2021年8月于北京市房山区第一医院接受CT增强及血管成像扫描的患者3600例为研究对象,调查碘对比剂外渗发生率,分析影响碘对比剂外渗的相关因素。结果:3600例CT增强及血管成像扫描患者中发生碘对比剂外渗24例,占比0.67%。多因素Logistic回归分析结果显示,年龄、合并基础病、注射速率、碘对比剂浓度、渗透压、注射护士工作时间是影响碘对比剂外渗的独立影响因素(P<0.05)。结论:CT增强及血管成像扫描中发生碘对比剂外渗的影响因素较多,因此临床需根据患者具体情况,选择合理的注射速率、碘对比剂浓度,加强医护人员操作技能专业培训,以减少碘对比剂外渗发生。 Objective:To analyze the influencing factors of iodine contrast agent extravasation during CT enhancement and angiography,and to discuss the intervention strategies.Methods:A total of three thousand and six hundred patients who received CT enhancement and angiography in Beijing Fangshan District First Hospital from January 2019 to August 2021 were selected as the study objects to investigate the incidence of iodine contrast agent extravasation.The related factors affecting the exosmosis of iodine contrast agent were analyzed.Results:Iodine contrast agent extravasation occurred in 24 cases(0.67%)of 3600 CT enhanced and angiographic scan patients.Multiple Logistic regression analysis showed that age,underlying diseases,injection rate,concentration of iodine contrast agent,osmotic pressure and working hours of injection nurses were independent factors affecting the extravasation of iodine contrast agent(P<0.05).Conclusion:There are many factors affecting iodide contrast extravasation in CT enhancement and angiography scanning.Therefore,it is necessary to select reasonable injection rate and iodide contrast concentration according to the specific conditions of patients,and strengthen professional training on operation skills of medical staff to reduce iodide contrast extravasation.
