全程多元化宣教模式联合人文关怀干预在晚期肝细胞癌患者中的应用效果

摘要 目的探讨全程多元化宣教模式联合人文关怀干预在晚期肝细胞癌(HCC)患者中的应用效果。方法根据干预方式的不同将104例晚期HCC患者分为对照组(n=52,常规干预)和观察组(n=52,全程多元化宣教模式联合人文关怀干预)。比较两组患者的负性情绪[焦虑自评量表(SAS)和抑郁自评量表(SDS)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)]、癌因性疲乏[简易疲乏量表(BFI)]、生活质量[欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)]、家属不确定感、家属满意度及不良反应发生情况。结果干预后,两组患者SAS、SDS、PSQI、BFI评分均低于本组干预前,EORTC QLQ-C30评分均高于本组干预前,观察组患者SAS、SDS、PSQI、BFI评分均低于对照组,EORTC QLQ-C30评分高于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者家属不确定感评分均低于本组干预前,观察组患者家属不确定感评分低于对照组,差异均有统计学意义(P﹤0.05)。观察组患者不良反应总发生率低于对照组,家属满意度高于对照组,差异均有统计学意义(P﹤0.05)。结论全程多元化宣教模式联合人文关怀干预能够改善晚期HCC患者的负性情绪,提高患者的睡眠质量、生活质量及家属满意度,缓解癌因性疲乏,降低不良反应发生率及家属不确定感。 Objective To explore the application effect of whole-course diversified education mode combined with humanistic care intervention in patients with advanced hepatocellular carcinoma(HCC).Method A total of 104 patients with advanced HCC were divided into control group(n=52,received conventional intervention)and observation group(n=52,received whole-course diversified education mode combined with humanistic care intervention)according to different intervention methods.The negative emotions[self-rating anxiety scale(SAS)and self-rating depression scale(SDS)],sleep quality[Pittsburgh sleep quality index(PSQI)],cancer-related fatigue[brief fatigue inventory(BFI)],quality of life[European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)],family members uncertainty,family members satisfaction and adverse reactions were compared between the two groups.Result After intervention,the scores of SAS,SDS,PSQI and BFI in both groups were lower than those before intervention,the EORTC QLQ-C30 scores were higher than those before intervention,the scores of SAS,SDS,PSQI and BFI in the observation group were lower than those in the control group,and the EORTC QLQ-C30 score was higher than that in the control group,and the differences were statistically significant(P<0.05).After intervention,the uncertainty scores of family members in both groups were lower than those before intervention,and the uncertainty score of family members in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and family members total satisfaction was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The whole-course diversified education mode combined with humanistic care intervention can improve the negative emotions of advanced HCC patients,improve the sleep quality,quality of life of patients and family members satisfaction,alleviate cancer-related fatigue,and reduce the incidence of adverse reactions and family members uncertainty.
