氨甲环酸对单髁置换术患者的影响

Effect of Tranexamic Acid on Patients Undergoing Monocondylar Replacement Surgery

目的:探讨氨甲环酸(Tranexamic Acid,TXA)对单髁置换术患者的影响。方法:选取2020年6月—2022年12月黄山新晨医院收治的58例单髁置换术患者。随机将其分为对照组(n=28)和观察组(n=30)。观察组给予TXA,对照组给予氯化钠注射液。比较两组围手术期指标、出血量、术后2 d、4 d、6 d的贫血指标、下肢深静脉血栓(deep vein thrombosis,DVT)。结果:观察组膝关节周围皮下瘀斑面积小于对照组,差异有统计学意义(P<0.05)。观察组术中失血量、隐性失血量均少于对照组,差异有统计学意义(P<0.05)。术后2 d、4 d、6 d,两组血红蛋白(hemoglobin,Hb)、血细胞比容(hematocrit,HCT)水平均升高,观察组Hb、HCT水平均高于对照组,差异有统计学意义(P<0.05)。两组下肢DVT发生率比较,差异无统计学意义(P>0.05)。结论:单髁置换术中使用TXA能减少患者出血量,改善贫血指标,且不会增加下肢DVT发生风险。

Objective:To investigate the effect of Tranexamic Acid(TXA)on patients with monocondylar replacement surgery.Method:A total of 58 patients with monocondylar replacement surgery admitted to Huangshan Xinchen Hospital from June 2020 to December 2022 were selected.They were randomly divided into control group(n=28)and observation group(n=30).The observation group was given TXA and the control group was given sodium chloride injection.Perioperative indexes,blood loss volume,anemia indexes on 2 d,4 d and 6 d after surgery,and lower limb deep vein thrombosis(DVT)were compared between the two groups.Result:The area of subcutaneous ecchymosis around the knee joint in the observation group was smaller than that in the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss volume and recessive blood loss volume in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).On 2 d,4 d and 6 d after surgery,the levels of hemoglobin(Hb)and hematocrit(HCT)were increased in both groups,and the levels of Hb and HCT in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of lower limb DVT between the two groups(P>0.05).Conclusion:The use of TXA during monocondylar replacement surgery can reduce blood loss and improve anemia markers without increasing the risk of lower limb DVT.
