腹腔镜胆囊切除术应用七氟烷和瑞芬太尼复合麻醉的效果分析

Effect Analysis of Sevoflurane and Remifentanil Combined Anesthesia in Laparoscopic Cholecystectomy

目的:分析腹腔镜胆囊切除术应用七氟烷和瑞芬太尼复合麻醉的效果。方法:选取2021年1月—2022年9月于泰安市东平县人民医院行腹腔镜胆囊切除手术的患者60例作为研究对象,依据随机数字表法分为两组,各30例。对照组给予盐酸瑞芬太尼联合丙泊酚维持麻醉,观察组应用七氟烷联合瑞芬太尼维持麻醉,比较两组麻醉效果。结果:观察组优良率高于对照组,差异有统计学意义(P=0.022)。气腹后5 min、气腹毕、手术结束时,两组收缩压、舒张压均高于麻醉前,但观察组低于对照组,差异有统计学意义(P<0.05)。观察组苏醒、拔管时间早于对照组,苏醒评分高于对照组,差异有统计学意义(P<0.001)。两组不良反应总发生率比较,差异无统计学意义(P=0.605)。结论:腹腔镜胆囊切除术应用七氟烷和瑞芬太尼复合麻醉的效果较好,患者生命体征相对平稳,恢复快,且麻醉安全性较好,值得推广。

Objective:To analyze the effect of sevoflurane and remifentanil combined anesthesia in laparoscopic cholecystectomy.Methods:A total of 60 patients undergoing laparoscopic cholecystectomy in Dongping County People's Hospital of Taian City from January 2021 to September 2022 were selected as the study subjects.They were divided into two groups according to random number table method,with 30 cases in each group.The control group was given remifentanil hydrochloride and propofol combined maintenance anesthesia and the observation group was given sevoflurane and remifentanil combined maintenance anesthesia.The anesthetic effect was compared between the two groups.Results:The excellent rate in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.022).The systolic blood pressure and diastolic blood pressure in the two groups at 5 min after pneumoperitoneum,at the end of pneumoperitoneum and at the end of the operation were higher than those before anesthesia,but these indexes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The awakening and extubation time in the observation group were earlier than those in the control group,and the awakening score in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).There was no significant difference in the total incidence of adverse reactions between the two groups(P=0.605).Conclusion:In laparoscopic cholecystectomy,the application effect of sevoflurane and remifentanil combined anesthesia has a good effect,the patient's vital signs are relatively stable with fast recovery and good anesthesia safety,which is worthy of promotion.
