基层社区老年人急性阑尾炎的治疗探究

Treatment of Acute Appendicitis in Elderly People in Grassroots Communities

目的:分析腹腔镜阑尾切除术治疗老年急性阑尾炎的效果。方法:选取2021年1—12月东营市利津县汀罗镇卫生院收治的急性阑尾炎老年患者94例作为研究对象,随机分为试验组与对照组,各47例。对照组实施传统开腹阑尾切除术,试验组实施腹腔镜阑尾切除术。比较两组手术效果。结果:试验组术中出血量少于对照组,手术时间及住院时间短于对照组,术后肛门排气时间早于对照组,差异有统计学意义(P<0.05)。治疗后,两组疼痛评分低于治疗前,且试验组低于对照组,差异有统计学意义(P<0.05)。试验组治疗总有效率高于对照组,差异有统计学意义(P=0.011)。试验组并发症总发生率低于对照组,差异有统计学意义(P=0.022)。治疗后,两组健康调查简表各项评分高于治疗前,且试验组高于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜阑尾切除术治疗老年急性阑尾炎的效果较好,患者临床指标及术后生活质量较优,疼痛程度轻,并发症发生率低。

Objective:To analyze the effect of laparoscopic appendectomy in the treatment of acute appendicitis in elderly people.Methods:A total of 94 elderly patients with acute appendicitis admitted to Dongying City Lijin County Tingluo Town Health Center from January 2021 to December 2021 were selected as the study subjects.They were randomly divided into an experimental group and a control group,with 47 cases in each group.The control group underwent traditional open appendectomy,while the experimental group underwent laparoscopic appendectomy.The surgical outcomes were compared between two groups.Results:The intraoperative blood loss in the experimental group was lower than that in the control group,the operation time and the length of hospital stay in the experimental group were shorter than those in the control group,the postoperative anal exhaust time in the experimental group was earlier than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the pain scores in the two groups were lower than those before treatment,and the scores in the experimental group were lower than those in the control group,with a statistically significant difference(P<0.05).The total effective rate of treatment in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.011).The total incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P=0.022).After treatment,the scores of the short form health survey in the two groups were higher than those before treatment,and the scores in the experimental group were higher than those in the control group,with a statistically significant difference(P<0.05).Conclusion:Laparoscopic appendectomy has a good effect in the treatment of acute appendicitis in elderly people.The clinical indicators and postoperative quality of life of patients are better,the degree of pain is mild,and the incidence of adverse reactions is low.
