腹腔镜阑尾切除术治疗急性阑尾炎效果及对患者炎症因子水平的影响

Effect of Laparoscopic Appendectomy for Acute Appendicitis and Influence on Levels of Inflammatory Factors of Patients

目的 探讨腹腔镜阑尾切除术对急性阑尾炎手术效果及炎症因子水平的影响。方法 将本院2019年1月—2023年7月收治的50例急性阑尾炎患者作为研究对象,随机分为试验组(n=25,采取腹腔镜阑尾切除术)和对照组(n=25,采取传统开腹阑尾切除术)。分别对比两组患者的手术指标及炎症因子水平。结果 试验组患者的切口长度、手术时长、首次排气时间、抗生素使用时间、住院时间均短于对照组,术中出血量、疼痛程度低于对照组(P<0.05);术前,两组患者的C反应蛋白(CRP)、白细胞介素6(IL-6)、降钙素原(PCT)水平无可比性(P>0.05);术后1d,两组患者的CRP、IL-6、PCT水平均高于术前,但试验组升高程度低于对照组(P<0.05);试验组并发症发生率低于对照组(P<0.05)。结论 腹腔镜阑尾切除术治疗急性阑尾炎疗效明确,有效减少炎症因子释放,术后并发症较少。

Objective To explore effect of laparoscopic appendectomy on surgical outcomes and levels of inflammatory factors of acute appendicitis.Methods The paper chose 50 patients with acute appendicitis admitted to our hospital from January 2019 to July 2023,and divided them into experimental group(n=25,treated with laparoscopic appendectomy) and control group(n=25,with traditional open appendectomy) randomly.Surgical indicators and levels of inflammatory factors were compared between two groups.Results Incision length,surgical duration,first exhaust time,antibiotic use time,and length of stay in experimental group was shorter than control group,intraoperative bleeding and pain level was lower than control group(P<0.05).Before surgery,there was no comparability in levels of C-reactive protein(CRP),interleukin-6(IL-6),and procalcitonin(PCT) between two groups(P>0.05).on the 1st day after surgery,levels of CRP,IL-6,and PCT in both groups were higher than before surgery,but elevation degree in experimental group was lower than control group(P<0.05).Incidence of complications in experimental group was 4.0%,lower than 24.0% in control group(P<0.05).Conclusion Laparoscopic appendectomy can achieve clear curative effect on acute appendicitis,can reduce release of inflammatory factors effectively,which has fewer postoperative complications.
