亚低温脑保护应用于重型创伤性颅脑损伤治疗的临床价值

摘要 目的 研究亚低温脑保护在重型创伤性颅脑损伤治疗中的应用效果。方法 纳入2022年8月—2023年8月本院收治的重型创伤性颅脑损伤患者展开研究。将所有的患者按照抽签法分成对照组和观察组,每组30例。对照组予以常规治疗,观察组则使用常规治疗+亚低温脑保护。就两组的治疗总有效情况、神经功能情况、直肠温度、颅内压、血糖指标等展开比较,并且对患者实施为期6个月的随访,观察、对比其预后情况。结果 连续治疗7d后,观察组治疗总有效率高于对照组(P<0.05)。治疗前,GCS评分、NIHSS评分和直肠温度、颅内压、血糖指标水平对比,两组差异无统计学意义(P>0.05);治疗后,观察组的GCS评分比对照组更高,但NIHSS评分、直肠温度、颅内压、血糖指标水平更低(P<0.05)。随访6个月后,观察组的预后良好率与对照组相比,处于更高的水平(P<0.05)。结论 将亚低温脑保护用在重型创伤性颅脑损伤中,能发挥出显著的作用,可以有效地降低体温、颅内压,使得脑部神经得到有效的保护,值得在临床广泛应用和推广。 Objective To study application effect of mild hypothermia brain protection in treatment of severe traumatic brain injury.Methods The paper chose 60 patients with severe traumatic brain injury admitted to our hospital from August 2022 to August 2023,and divided them into control group and observation group by drawing lots,with 30 cases in each group.Control group was treated with routine treatment,while observation group with routine treatment+mild hypothermia brain protection.Total effect of treatment,neurological function,rectal temperature,intracranial pressure,blood glucose indicators,etc.were compared between two groups,prognosis was observed and compared after 6-month follow-up.Results After 7 days of continuous treatment,total effective rate of observation group was higher than control group(P<0.05).Before treatment,there was no significant difference for comparison of GCS scores,NIHSS scores,rectal temperature,intracranial pressure,and blood glucose levels between two groups,(P>0.05).After treatment,GCS scores of observation group were higher than control group,NIHSS scores,rectal temperature,intracranial pressure,and blood glucose levels were lower(P<0.05).After 6 months of follow-up,good prognosis rate of observation group was at a higher level than control group(P<0.05).Conclusion Hypothermic brain protection can achieve significant effect for in severe traumatic brain injury,reduce body temperature and intracranial pressure effectively,and provide effective protection for brain nerves,which is worthy of widespread clinical application and promotion.
