磁共振成像在慢性阻塞性肺疾病合并肺动脉高压患者肺动脉血流和右心功能评估中的应用价值

Application Value of MRI in Evaluating Pulmonary Artery Flow and Right Heart Function in Patients with Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Hypertension

目的研究磁共振成像(Magnetic Resonance Imaging,MRI)用于慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)合并肺动脉高压患者中,对其肺动脉血流以及右心功能的评估效果。方法随机选取2021年1月—2023年1月湖北省中医院收治的60例COPD患者为对象,以患者有无肺动脉高压分组,其中有肺动脉高压者25例,为观察组;无肺动脉高压者35例,为对照组。两组均行MRI检查,选择检查结果中肺动脉血流3项指标和右心室4项功能指标进行比较分析。结果观察组主肺动脉血流3项指标中,血流量平均值(59.88±12.64)mL、峰值(52.53±15.81)mL/cm、顺应性(22.06±6.97)%,低于对照组,差异有统计学意义(t=6.562、4.707、6.698,P均<0.001)。观察组右心功能4项指标中,收缩末期容积、舒张末期容积高于对照组,每搏输出量(Stroke Volume,SV)、射血分数(Ejection Fraction,EF)低于对照组,差异有统计学意义(P均<0.05)。结论MRI能有效检测出COPD合并肺动脉高压患者的肺动脉血流和右心功能指标异常情况,对提升两者评估价值有积极作用。

Objective To investigate the effect of magnetic resonance imaging(MRI)on pulmonary artery flow and right heart function in patients with chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension.Methods A total of 60 COPD patients treated in Hubei Provincial Hospital of Tradi⁃tional Chinese Medicine from January 2021 to January 2023 were randomly selected as subjects.The patients were grouped with and without pulmonary arterial hypertension,with 25 cases of pulmonary arterial hyperten⁃sion as the observation group,and 35 cases without pulmonary arterial hypertension were selected as the con⁃trol group.MRI examination was performed in both groups.3 indexes of pulmonary artery blood flow and 4 func⁃tional indexes of right ventricle were selected for comparative analysis.Results The mean value of blood flow(59.88±12.64)mL,the peak value(52.53±15.81)mL/cm,and the compliance(22.06±6.97)%in the observation group were lower than those in the control group,and the differences were statistically significant(t=6.562,4.707,6.698,all P<0.001).The end-systolic volume and end-diastolic volume in the observation group were higher than those in the control group,and stroke volume(SV)and ejection fractions(EF)were lower than those in control group,and the differences were statistically significant(all P<0.05).Conclusion MRI can effectively detect the abnormal pulmonary artery flow and right heart function indicators in COPD patients with pulmonary hypertension,and has a positive effect on improving the evaluation value of both.
