PT、APTT、D-Dimer与妊娠期糖尿病患者血糖水平的相关性研究

Study on the Correlation between PT,APTT,D-Dimer and Blood Glucose Levels in Patients with Gestational Diabetes Mellitus

目的分析孕期凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-Dimer)与妊娠期糖尿病患者血糖水平的相关性。方法选取2020年10月至2022年10月收治的妊娠期糖尿病患者60例作为患者组,另选取同期在院体检的健康孕妇60例作为健康组,测量凝血功能指标及血糖水平,进行妊娠期糖尿病患者凝血功能指标与血糖水平的相关性分析,比较两组的妊娠结局。结果患者组D-Dimer、FPG、2hPG水平高于健康组,PT、APTT短于健康组(P<0.05)。妊娠期糖尿病患者PT、APTT与FPG、2hPG水平呈负相关(r=-0.364、-0.563、-0.426、-0.631,P<0.05),D-Dimer与FPG、2hPG水平呈正相关(r=0.341、0.482,P<0.05)。患者组不良妊娠结局发生率高于健康组(P<0.05)。结论妊娠期糖尿病患者孕期凝血功能指标与健康孕妇有明显差异,其血糖水平与凝血功能密切相关,高血凝状态易增加患者不良妊娠结局风险。

Objective To analyze the correlation between prothrombin time(PT),activated partial thromboplastin time(APTT),D-Dimer and blood glucose levels in patients with gestational diabetes mellitus.Methods 60 patients with gestational diabetes mellitus from October 2020 to October 2022 were selected as patient group,and 60 healthy pregnant women with physical examination at the same time were selected as healthy group.The coagulation function indicators and blood glucose levels were detected,and the correlation between coagulation function indicators and blood glucose levels of patients with gestational diabetes mellitus was analyzed.The pregnancy outcome was compared between the two groups.Results The D-Dimer,FPG and 2hPG levels of the patient group were higher than those of the healthy group,and the PT and APTT were shorter than those of the healthy group(P<0.05).Among patients with gestational diabetes mellitus,PT and APTT were negatively correlated with FPG and 2hPG(r=-0.364,-0.563,-0.426,-0.631,P<0.05),while D-Dimer was positively correlated with FPG and 2hPG(r=0.341,0.482,P<0.05).The incidence of adverse pregnancy outcomes in the patient group was higher than that in the healthy group(P<0.05).Conclusions The coagulation function indicators in patients with gestational diabetes mellitus are significantly different from those of healthy pregnant women.The blood glucose level is closely correlated with the coagulation function.High blood coagulation status tends to increase the risk of adverse pregnancy outcomes.
