Vitapex糊剂根管填充治疗儿童乳牙牙髓炎的疗效及对IL-1β、IL-18水平的影响

The Therapeutic Effect of Vitapex Paste Root Canal Filling in the Treatment of Pediatric Primary Pulpitis and its Impact on IL-1β and IL-18 Levels

【目的】探讨Vitapex糊剂根管填充对儿童乳牙牙髓炎的疗效及对白细胞介素-1β(IL-1β)、白细胞介素-18(IL-18)水平的影响。【方法】选取2020年3月至2022年3月在西安市第三医院诊治的92例乳牙牙髓炎患儿,按照随机数字表法分为观察组(n=46,患牙52颗)和对照组(n=46,患牙56颗)。观察组采用Vitapex糊剂根管填充治疗,对照组采用氧化锌丁香油糊剂根管填充治疗。比较两组患儿肿胀消失时间、疼痛缓解时间、牙齿疼痛程度、根管治疗成功率及IL-1β、IL-18水平。【结果】观察组肿胀消失时间、疼痛缓解时间短于对照组(P<0.05)。治疗前,两组IL-1β、IL-18水平比较,差异无统计学意义(P>0.05);治疗后,两组IL-1β、IL-18水平均降低(P<0.05),且观察组低于对照组(P<0.05)。术后2 d,两组视觉模拟评分法(visual analogue scale,VAS)评分较术前均显著降低(P<0.05),且观察组低于对照组(P<0.05);术后3 d,两组VAS评分较术前及术后2 d均显著降低,且观察组低于对照组(P<0.05)。随访1年,对照组根尖发炎10颗,治疗成功率82.14%(46/56);观察组根尖发炎2颗,治疗成功率96.15%(50/52)。观察组根管治疗成功率高于对照组,差异有统计学意义(χ^(2 )=5.359,P=0.021)。【结论】Vitapex糊剂根管填充治疗乳牙牙髓炎患儿,疗效显著,可降低IL-1β、IL-18水平,减轻患儿术后牙体疼痛程度,提高根管治疗成功率。

【Objective】To explore the therapeutic effect of Vitapex paste root canal filling on pulpitis of primary teeth in children and its impact on the levels of interleukin-1β(IL-1β)and interleukin-18(IL-18).【Methods】A total of 92 children with primary pulpitis treated at the Third Hospital of Xi'an City from March 2020 to March 2022 were selected and randomly divided into an observation group(n=46,52 affected teeth)and a control group(n=46,56 affected teeth)using a random number table method.The observation group received root canal filling with Vitapex paste,while the control group received treatment with zinc oxide clove oil paste.We compared the swelling disappearance time,pain relief time,degree of tooth pain,success rate of root canal treatment,and IL-1βand IL-18 levels between two groups of pediatric patients.【Results】The swelling disappearance time and pain relief time in the observation group were shorter than those in the control group(P<0.05).Before treatment,there was no statistically significant difference in IL-1βand IL-18 levels between the two groups(P>0.05);After treatment,the levels of IL-1βand IL-18 in both groups decreased(P<0.05),and the observation group was lower than the control group(P<0.05).2 days after surgery,the visual analog scale(VAS)scores of both groups were significantly lower than before surgery(P<0.05),and the observation group was lower than the control group(P<0.05);After 3 days of surgery,the VAS scores of both groups significantly decreased compared to preoperative and postoperative 2 days,and the observation group was lower than the control group(P<0.05).Follow up for 1 year showed that in the control group,10 apical inflammations occurred,with a treatment success rate of 82.14%(46/56);Two apical inflammations were observed in the observation group,with a treatment success rate of 96.15%(50/52).The success rate of root canal treatment in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2 )=5.359,P=0.021).【Conclusion】Vitapex paste root canal filling has a significant therapeutic effect on children with primary pulpitis and can reduce IL-1βand IL-18 levels,alleviate postoperative tooth pain in pediatric patients and improve the success rate of root canal treatment.
