“虚火理论”在脂溢性皮炎诊疗中的应用

摘要 脂溢性皮炎是临床常见皮肤病,主要表现为红斑伴油腻鳞屑,影响患者生活质量。学界普遍认为脂溢性皮炎是因内蕴湿热,外感风邪,蕴阻肌肤而发病,治疗多用清热利湿法。笔者认为目前临床所见脂溢性皮炎,常因熬夜、情志不畅等不良生活习惯诱发或加重,结合现代人体质特点及临床经验,提出可从“虚火”角度认识脂溢性皮炎,辨清本虚标实的病机,治疗时分经、分脏腑对应处方用药,同时也为脂溢性皮炎的炎症反应发生机制提供较为合理的解释。笔者以此思路治疗脂溢性皮炎效果显著,故将“虚火理论”在脂溢性皮炎诊疗中的应用介绍如下,并附验案一则辅助阐明。 Seborrheic dermatitis is a common skin disease in clinical practice,mainly characterized by erythema accompanied by greasy scales,affecting the quality of life of patients.The academic community generally believes that seborrheic dermatitis is caused by intrinsic damp-heat and external wind-evil,which blocks the muscles and skin and causes illness.The treatment mostly uses the method of clearing heat and dampness.The author believes that the seborrheic dermatitis currently seen in clinical practice is often induced or aggravated by bad habits such as staying up late and emotional discomfort.Combining the characteristics of modern physique and clinical experience,it is proposed to understand seborrheic dermatitis from the perspective of“virtual fire”,distinguish the pathogenesis of the real and the false,treat the prescription according to the meridians and organs during treatment,and also provide a more reasonable explanation for the inflammatory response mechanism of seborrheic dermatitis.The author has achieved significant results in treating seborrheic dermatitis with this idea,so the application of the“virtual fire theory”in the diagnosis and treatment of seborrheic dermatitis is introduced below,and a case is attached to assist in clarification.
