针刺联合畲药逐痰通络方治疗痰瘀阻络型高血压病伴颈动脉硬化临床研究

Clinical Study on Acupuncture Combined with Zhutan Tongluo Decoction of Traditional She Medicine for Hypertension with Carotid Arteriosclerosis of PhlegmStasis Obstructing Collaterals Type

目的:观察针刺联合畲药逐痰通络方治疗痰瘀阻络型高血压病伴颈动脉硬化患者的临床疗效。方法:选取60例痰瘀阻络型高血压病伴颈动脉硬化患者,按照随机数字表法分成对照组和治疗组各30例。对照组给予常规西药治疗,治疗组在对照组基础上加用针刺联合畲药逐痰通络方治疗,疗程均为4周。比较2组治疗前后中医证候积分、血压、颈动脉内中膜厚度(IMT)、肱踝脉搏波传导速度(baPWV)及踝臂指数(ABI)水平,并比较2组临床疗效。结果:治疗组总有效率93.33%,高于对照组73.33%(P<0.05)。治疗后,2组中医证候积分、舒张压(DBP)、收缩压(SBP)、IMT、baPWV均降低(P<0.05),ABI均升高(P<0.05),且治疗组中医证候积分、DBP、SBP、IMT、baPWV均低于对照组(P<0.05),ABI高于对照组(P<0.05)。结论:针刺联合畲药逐痰通络方能改善痰瘀阻络型高血压病伴颈动脉硬化斑块患者的临床疗效,降低患者血压,改善血管弹性,缓解临床症状。

Objective:To observe the clinical effect of acupuncture combined with Zhutan Tongluo Decoction of traditional She medicine on hypertension with carotid arteriosclerosis of phlegm-stasis obstructing collaterals type.Methods:A total of 60 patients with hypertension with carotid arteriosclerosis of phlegm-stasis obstructing collaterals type were selected and divided into the control group and the treatment group according to the random number table method,with 30 cases in each group.The control group was given conventional western medicine,and the treatment group was treated with acupuncture combined with Zhutan Tongluo Decoction of traditional She medicine based on the treatment of the control group.Both groups were treated for 4 weeks.The levels of traditional Chinese medicine(TCM) syndrome scores,blood pressure,carotid intima-media thickness(IMT),brachial-ankle pulse wave velocity(baPWV) and ankle-brachial index(ABI) were compared before and after treatment,and the clinical effects in the two groups were also compared.Results:The total effective rate was 93.33% in the treatment group,higher than that of 73.33% in the control group(P<0.05).After treatment,the TCM syndrome scores,diastolic blood pressure(DBP),systolic blood pressure(SBP),IMT,baPWV in both groups were significantly decreased(P<0.05),and ABI levels were significantly increased(P<0.05).Compared with those in the control group after treatment,the TCM syndrome scores,DBP,SBP,IMT,baPWV in the treatment group were significantly decreased(P<0.05),and ABI was significantly increased(P<0.05).Conclusion:Acupuncture combined with Zhutan Tongluo Decoction of traditional She medicine can improve the clinical effect on patients with hypertension with carotid arteriosclerosis of phlegm-stasis obstructing collaterals type,reduce blood pressure,improve vascular elasticity,and relieve clinical symptoms.
