25G玻璃体切除术治疗增生性糖尿病视网膜病变对患者视力及眼压水平 炎症反应指标的影响

Effects of 25G vitrectomy for proliferative diabetes retinopathy on visual acuity, intraocular pressure level and inflammatory response indicators

目的探讨25G玻璃体切除术治疗增生性糖尿病视网膜病变(PDR)对患者视力及眼压水平、炎症反应指标的影响。方法选取2020年6月至2023年5月本院收治的PDR患者82例为研究对象,将82例PDR患者按照随机数字表法分为2组。对照组(41例)给予23G玻璃体切除术治疗,观察组(41例)实施25G玻璃体切除术治疗。对比分析2组患者的视力、眼压水平、炎症反应、并发症。结果观察组的眼压水平(13.1±2.6)mmHg、并发症5%均低于对照组的(15.2±3.7)mmHg、27%,差异有统计学意义(P<0.05);观察组炎症反应总发生率为10%(4/41),与对照组15%(6/41)相当,差异无统计学意义(P>0.05);观察组的低视力患者生活质量调查量表(CLVQOL)中的远视力、移动和光感(79.9±4.6)分、日常生活能力(83.2±5.8)分、阅读和精细动作能力(82.1±5.5)分、调节能力(84.1±5.0)分均高于对照组的(70.5±2.4)分、(72.9±3.7)分、(69.8±3.5)分、(73.6±4.0)分,差异有统计学意义(P<0.05)。结论在PDR患者中应用25G玻璃体切除术效果显著,能够有效改善患者的眼压水平以及视力,提高其生活质量,且不增加炎症反应,并发症少,安全性高。

Objective To investigate the effect of 25G vitrectomy on the visual acuity,intraocular pressure,and inflammatory response in patients with proliferative diabetes retinopathy(PDR).Methods A total of 82 PDR patients admitted to the hospital from June 2020 to May 2023 were selected as the study subjects.The 82 PDR patients were randomly divided into two groups using a random number table method.The control group(41 cases)received 23G vitrectomy treatment,while the observation group(41 cases)received 25G vitrectomy treatment.Compare and analyze the visual acuity,intraocular pressure levels,inflammatory reactions,and complications of the two groups of patients.Results The intraocular pressure level(13.1±2.6)mmHg,and complications(5%)of the observation group were lower than those of the control group(15.2±3.7)mmHg,and 27%,with statistical significance(P<0.05).The total incidence of inflammatory reactions in the observation group was 10%(4/41),which was equivalent to 15%(6/41)in the control group,with no statistically significant difference(P>0.05).The quality-of-life questionnaire(CLVQOL)for low vision patients in the observation group showed higher scores in distant vision,movement and light perception(79.9±4.6),daily living ability(83.2±5.8),reading and fine motor ability(82.1±5.5),and regulatory ability(84.1±5.0)compared to the control group(70.5±2.4),(72.9±3.7),(69.8±3.5),and(73.6±4.0),with statistically significant differences(P<0.05).Conclusion The application of 25G vitrectomy in PDR patients has a significant effect.It can effectively improve the patients′intraocular pressure level and vision,improve their quality of life,and not increase inflammatory reactions,with fewer complications and high safety.
