分析咪达唑仑联合右美托咪定复合气管插管麻醉在甲状腺切除术中的麻醉价值

Analysis of the Anesthetic Value of Midazolam Combined with Dexmedeto⁃midine Compounded with Tracheal Intubation Anesthesia in Thyroidec⁃tomy

目的研究咪达唑仑联合右美托咪定复合气管插管麻醉运用于甲状腺切除术患者中的效果。方法选取2020年8月—2023年8月无锡市新吴区新瑞医院接诊的80例甲状腺切除术患者(均接受气管插管麻醉)为研究对象,按随机数表法分为两组,各40例,分别开展右美托咪定麻醉(对照组)与咪达唑仑联合右美托咪定麻醉(观察组),对比两组的麻醉效果。结果与对照组相比,观察组一次性插管成功率更高,镇静达标时间更短,差异有统计学意义(P均<0.05)。观察组(给药后至术毕时)Ramsay评分更高,差异有统计学意义(P<0.05)。观察组(给药后至插管后)心率更低,平均动脉压更高,差异有统计学意义(P均<0.05)。观察组(插管前至插管后)去甲肾上腺素、肾上腺素更低,差异有统计学意义(P均<0.05)。观察组不良反应发生率(5.00%)低于对照组,差异有统计学意义(χ^(2)=4.114,P<0.05)。结论咪达唑仑联合右美托咪定复合气管插管麻醉运用于甲状腺切除术中的安全性高,可以降低对应激指标的影响。

Objective To study the effect of midazolam combined with dexmedetomidine compound tracheal intubation anesthesia used in patients undergoing thyroidectomy.Methods A total of 80 thyroidectomy patients(all received tracheal intubation anesthesia)treated in Wuxi Xinwu District Xinrui Hospital from August 2020 to August 2023 were selected as the study object,and they were divided into two groups by random number table method,with 40 cases in each group,they were implemented anesthesia with dexmedetomidine(control group)and midazolam combined with dexmedetomidine(observation group)respectively.The anesthetic effects were compared between the two groups.Re⁃sults Compared with the control group,the observation group had a higher success rate of one-time intubation and a shorter time of sedation attainment,and the differences were statistically significant(both P<0.05).The Ramsay scores of the observation group(after drug administration to the end of surgery)were higher,the difference was statistically significant(P<0.05).Heart rate were lower and mean arterial pressure were higher in the observation group(after drug administration to after intubation),and the differences were statistically significant(both P<0.05).Norepinephrine and epinephrine were lower in the observation group(before intubation to after intubation),and the differences were statistically significant(all P<0.05).The adverse reaction rate of 5.00%was lower in the observation group than that in the control group,and the differences were statistically significant(χ^(2)=4.114,P<0.05).Conclusion Midazolam combined with dexmedetomidine compound tracheal intubation anesthesia used in thyroidectomy is safe,can reduce the impact on stress indicators.
