不同病原体呼吸道感染患儿外周血淋巴细胞亚群差异对比

Comparison of Lymphocyte Subsets in Peripheral Blood of Children with Respiratory Tract Infection with Different Pathogens

摘要 目的对比分析不同病原体呼吸道感染患儿外周血淋巴细胞亚群指标的差异。方法选取2022年6月—2023年6月宜兴市人民医院接诊收治的120例呼吸道感染患儿纳入观察组,其中包含甲流患儿60例,支原体肺炎患儿60例,选择同期进行体检的健康儿童60例纳入对照组。对比观察组与对照组外周血淋巴细胞亚群指标的检测结果,进一步比较不同病原体患儿各项淋巴细胞亚群指标差异。结果观察组呼吸道感染患儿CD3^(+)(907.48±11.15)%、CD3^(+)CD8^(+)(254.94±10.13)%、CD3-CD19^(+)(89.51±5.22)%与CD3-CD56/16^(+)(98.85±10.20)%均明显低于对照组,CD3-CD4^(+)(2307.48±12.29)%高于对照组,差异有统计学意义(t=123.749、177.729、51.590、43.580、765.587,P均<0.05)。结合观察组患儿不同病原体类型检测结果分析显示,甲流(病毒感染)患儿CD3^(+)、CD3^(+)CD8^(+)、CD3-CD19^(+)与CD3-CD56/16^(+)均低于支原体肺炎(细菌感染)患儿,CD3-CD4^(+)高于支原体肺炎患儿,差异有统计学意义(P均<0.05)。结论呼吸道感染患儿不同病原体感染所引起外周血淋巴细胞亚群测定结果存在明显差异,临床可通过检测该指标用于掌握儿童免疫状态,判断感染类型,为进一步治疗提供依据。 Objective To analyze the difference of lymphocyte subsets of peripheral blood detected with different pathogens in children with respiratory tract infection.Methods A total of 120 children with respiratory tract infection admitted to Yixing People's Hospital from June 2022 to June 2023 were included in the observation group,including 60 children with H1N1 influenza and 60 children with mycoplasma pneumonia,and 60 healthy children who under went physical examination during the same period were included in the control group.The detection results of periph eral blood lymphocyte subsets in the observation group and the control group were compared,and the differences of various lymphocyte subsets in children with different pathogens were further compared.Results CD3^(+)(907.48±11.15)%,CD3^(+)CD8^(+)(254.94±10.13)%,CD3-CD19^(+)(89.51±5.22)%and CD3-CD56/16^(+)(98.85±10.20)%of chil dren with respiratory tract infection in observation group were significantly lower than those in control group,and CD3-CD4^(+)(2307.48±12.29)%was higher than that of control group,and the differences were statistically significant(t=123.749,177.729,51.590,43.580,765.587,all P<0.05).Combined with the detection results of different pathogen types in the observation group,the results showed that CD3^(+),CD3^(+)CD8^(+),CD3-CD19^(+)and CD3-CD56/16^(+)of the chil dren with influenza A(virus infection)were lower than those with mycoplasma pneumonia(bacterial infection),and CD3-CD4^(+)was higher than that with mycoplasma pneumonia,and the differences were statistically significant(all P<0.05).Conclusion There are obvious differences in the determination results of peripheral blood lymphocyte subsets caused by different pathogens in children with respiratory tract infection.Clinically,this index can be used to grasp the immune status of children and determine the type of infection,so as to provide evidence for further treatment.
