甘温除热法联合穴位针刺对妇产科手术后发热患者发热时程及抗生素使用情况的影响观察

Effects of Relieving Fever with Sweet and Warm Drugs Method Combined with Acupoint Acupuncture on Fever Course and Antibiotics Use in Patients with Fever after Gynecologic Surgery

目的观察甘温除热法联合穴位针刺对妇产科手术后发热患者发热时程及抗生素使用情况的影响。方法选取2020年8月—2022年8月该院收治的100例妇产科手术后发热患者为研究对象,按随机数字法分为对照组和观察组各50例,两组均给予抗生素、补液、营养支持治疗,对照组给予甘温除热法治疗,观察组在对照组基础上给予穴位针刺治疗,比较两组临床疗效、发热时程、抗生素使用情况、炎症因子指标[白细胞计数(white blood cell count,WBC)、中性粒细胞(neutrophil,N)、C反应蛋白(C-reactive protein,CRP)、降钙素原(calcitonin,PCT)]、免疫功能指标[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]及不良反应发生率。结果观察组治疗有效率高于对照组[96.00%(48/50)vs 80.00%(40/50)](P<0.05)。观察组发热时程短于对照组,≥2种抗生素使用率低于对照组(P<0.05)。治疗后两组WBC、N、CRP、PCT均降低,血清IgA、IgG、IgM水平均升高(P<0.05),且观察组WBC、N、CRP、PCT显著低于对照组,血清IgA、IgG、IgM水平显著高于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论甘温除热法联合穴位针刺治疗妇产科手术后发热患者疗效显著,可缩短发热时程,减少抗生素使用,降低炎症因子水平,提高免疫功能。

Objective To observe the effects of relieving fever with sweet and warm drugs method combined with acupoint acupuncture on fever course and antibiotics use in patients with fever after gynecologic surgery.Methods A total of 100 patients with fever after gynecologic surgery admitted to the hospital were enrolled as the research objects between August 2020 and August 2022.According to random number table method,they were divided into control group and observation group,50 cases in each group.After treatment with antibiotics,fluid infusion and nutrition support were used in both groups.The control group was treated with relieving fever with sweet and warm drugs method,while the observation group was additionally given acupoint acupuncture.The clinical curative effect,fever course,antibiotics use,inflammatory factors[white blood cell count(WBC),neutrophil(N),C-reactive protein(CRP),calcitonin(PCT)],immune function indexes[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)]and incidence of adverse reactions were compared between the two groups.Results The response rate in the observation group was higher than that in the control group[96.00%(48/50)vs 80.00%(40/50)](P<0.05).The fever course in the observation group was shorter than that in the control group,and the use rate of dual antibiotics was lower than that in the control group(P<0.05).After treatment,the values of WBC,N,CRP and PCT were decreased,while the levels of serum IgA,IgG and IgM were increased in both groups(P<0.05).The values of WBC,N,CRP and PCT in the observation group were significantly lower than those in the control group,while the levels of serum IgA,IgG and IgM were significantly higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The curative effect of relieving fever with sweet and warm drugs method combined with acupoint acupuncture is significant on patients with fever after gynecologic surgery,which can shorten fever course,reduce antibiotics use,reduce the levels of inflammatory factors and enhance immune function.
