二维联合彩色多普勒超声对甲状腺癌的诊断价值研究

摘要 目的分析在甲状腺癌诊断中实施二维联合彩色多普勒超声检查的价值。方法选取2022年9月至2023年8月在本院进行诊治的疑似甲状腺癌患者89例为研究对象,所有均予以患者超声检查,金标准为病理诊断,对比二维超声单项诊断、彩色多普勒超声单项诊断和联合诊断甲状腺癌的效能,比较良性组与恶性组患者二维声像图回声及彩色多普勒超声参数。结果相比于二维超声,彩色多普勒超声诊断甲状腺癌的灵敏度和准确度相对更高(χ^(2)=3.56,4.49,P均<0.05),2种检查方式诊断甲状腺癌的特异度以及阳性预测值、阴性预测值差异无统计学意义(χ^(2)=0.98,1.16,2.51,P均>0.05)。与彩色多普勒相比,在甲状腺癌诊断中实施二维联合彩色多普勒超声检查甲状腺癌具有更高的诊断灵敏度和准确度(χ^(2)=4.17,5.63,P均<0.05),诊断特异度、阳性预测值、阴性预测值差异无统计学意义(χ^(2)=1.21,1.33,0.97,P均>0.05)。强回声与混合回声比较,良性组与恶性组差异无统计学意义(χ^(2)=0.40,3.00,P均>0.05),恶性组低回声检出率高于良性组,等回声检出率低于良性组,差异有统计学意义(χ^(2)=14.07,37.51,P均<0.05)。相比于良性组,恶性组具有更高的阻力指数(RI)值和收缩期峰值流速(Vmax)值(t=3.62,4.70,P均<0.05)。结论二维超声及彩色多普勒超声在甲状腺癌诊断中均有一定的应用价值,联合应用二维超声及彩色多普勒超声可进一步提高诊断效能。 Objective To study and analyze the value of two-dimensional(2D)ultrasound which is combined with color Doppler ultrasound in thyroid cancer.Methods Eighty-nine patients with suspected thyroid diagnosis and treatment from September 2022 to August 2023 were selected and given an ultrasound examination.Taking the pathological diagnosis as the gold standard,the diagnostic efficacy in thyroid cancer with the 2D ultrasound examination,color Doppler ultrasound examination,and combined examination were compared and analyzed,and the echo and color Doppler ultrasound parameters between the benign group and malignant group were compared.Results Compared to 2D ultrasound,color Doppler ultrasound had relatively higher sensitivity and accuracy in diagnosing thyroid cancer(χ^(2)=3.56,4.49,all P<0.05).There was no significant difference in the specificity,positive predictive value,and negative predictive value of the two diagnostic methods for thyroid cancer(χ^(2)=0.98,1.16,2.51,all P>0.05).Compared with color Doppler,the implementation of 2D ultrasound combined color Doppler ultrasound in the diagnosis of thyroid cancer had higher diagnostic sensitivity and accuracy(χ^(2)=4.17,5.63,all P<0.05),and there was no significant difference in diagnostic specificity,positive predictive value,and negative predictive value(χ^(2)=1.21,1.33,0.97,all P>0.05).The comparison of the proportion of strong echo and mixed echo shows that there is no significant difference between the benign group and the malignant group(χ^(2)=0.40,3.00,all P>0.05),the low echogenicity detection rate in the malignant group was higher than that in the benign group,and the iso-echogenicity detection rate was lower than that in the benign group,with statistically significant differences(χ^(2)=14.07,37.51,all P<0.05).Compared to the benign group,the malignant group had higher RI and Vmax values(t=3.62,4.70,all P<0.05).Conclusion 2D ultrasound and color Doppler ultrasound have some application value in thyroid cancer diagnosis,and combining 2D ultrasound can further improve diagnostic efficiency.
