利湿健脾祛瘀汤在西医基础上对口腔扁平苔藓疗效及对血清肿瘤坏死因子-α、白细胞介素-6、白细胞介素-8水平的影响分析

An analysis of the effect of Lishi Jianpi Quyu decoction plus Western medicine on oral lichen planus and serum TNF-α,IL-6 and IL-8 levels

目的:探析在西医基础上使用利湿健脾祛瘀汤治疗口腔扁平苔藓的效果及对患者肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-8(Interleukin-8,IL-8)等水平的影响。方法:选取2020年3月―2021年5月青州市中医院口腔科收治的口腔扁平苔藓患者84例,采用随机数字表法将其分为观察组与对照组,每组42例,对照组患者接受常规西医治疗,观察组患者在此基础上加用利湿健脾祛瘀汤治疗,比较两组临床疗效、治疗前后症状严重程度(网纹-糜烂-溃疡评分)、不适程度[视觉模拟评分法(Visual Analogue Scale,VAS)评分]及血清炎症因子(TNF-α、IL-6、IL-8)水平。结果:与对照组比较,观察组疗效明显更高,差异有统计学意义(P <0.05);治疗前,观察组与对照组患者的网纹-糜烂-溃疡评分、VAS评分、TNF-α、IL-6、IL-8等指标的差异均无统计学意义(P> 0.05),治疗1个月后,与对照组比较,观察组网纹-糜烂-溃疡评分、VAS评分、TNF-α、IL-6、IL-8等指标水平明显更低,差异有统计学意义(P <0.05)。结论:对口腔扁平苔藓患者在西医基础上使用利湿健脾祛瘀汤治疗可获得较好的效果,能够有效改善病损,降低患者不适程度及炎症损伤,同时还能有效降低疼痛程度,相较于常规治疗措施更能促进治疗后康复,具有较高的临床推广价值。

Objective:To analyze the effect of Western medicine plus Lishi Jianpi Quyu decoction(利湿健脾祛瘀汤)on oral lichen planus and its effect on tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-8(IL-8)of patients.Methods:A total of 84 patients with oral lichen planuss treated in Department of Stomatology,Qingzhou Hospital of Traditional Chinese Medicine from March 2020 to May 2021 were selected and divided into the observation group the control group by random number table method,with 42 patients in each group.The patients in the control group received conventional Western medicine treatment,and patients in the observation group were treated with Lishi Jianpi Quyu decoction on the basis of Western medicine treatment.The clinical efficacy,symptom severity[(Reticulation-erosion-ulcer,REU)score],discomfort[visual analogue scale(VAS)]and serum inflammatory cytokines(TNF-α,IL-6,IL-8)before and after treatment were compared between the two groups.Results:Compared with the control group,the effect in the observation group was significantly higher(P<0.05).Before treatment,there were no significant differences in REU score,VAS score,TNF-α,IL-6,IL-8 and other indexes between the observation group and the control group(P>0.05).After 1 month of treatment,compared with those in the control group,REU score,VAS score,TNF-α,IL-6,IL-8 and other indexes in the observation group were significantly lower,and the difference was statistically significant(P<0.05).Conclusion:Lishi Jianpi Quyu decoction can get a good effect in treating patients with oral lichen planus on the basis of Western medicine treatment.It can effectively relieve the lesions,reduce the degree of discomfort and inflammatory damage of patients,and also effectively reduce the degree of pain.Compared with conventional treatment measures,it can promote the rehabilitation after treatment,and has a high clinical promotion value.
