摘要
目的探讨磁共振血管壁成像(VWI-MRI)评估颅内未破裂动脉瘤稳定性的价值。方法选取2021年1月—2023年1月就诊的120例颅内未破裂动脉瘤,根据动脉瘤稳定性将其分为稳定组(n=64)和不稳定组(n=56),均行VWI-MRI检查,比较2组及5年内不同生长、破裂风险未破裂动脉瘤患者VWI-MRI参数,分析VWI-MRI参数与颅内未破裂动脉瘤稳定性和5年内生长、破裂风险相关性。结果稳定组与不稳定组、5年内生长风险低高危和破裂风险低高危未破裂动脉瘤患者是否为分叉动脉瘤、形态、强化形式、瘤壁强化分级和强化指数比较差异具有统计学意义(P<0.05,P<0.01);颅内未破裂动脉瘤稳定性和5年内生长、破裂风险与分叉动脉瘤、形态、强化形式、瘤壁强化分级和强化指数均呈正相关(P<0.01)。结论VWI-MRI检查可用于评估颅内未破裂动脉瘤稳定性,并为临床早期评估动脉瘤生长、破裂风险提供参考。
Objective To investigate the value of vessel wall magnetic resonance imaging(VWI-MRI)in the stability of unruptured intracranial aneurysms(UIA).Methods A total of 120 patients with UIA admitted to our hospital from January 2021 to January 2023 were selected and divided into stable group(n=64)and unstable group(n=56)according to the stability of aneurysms.VWI-MRI examinations were performed,and VWI-MRI parameters were compared between the two groups and patients with different growth and rupture risk within 5 years[location,forked aneurysm,shape,form of enhancement,wall enhancement grade,enhancement index,tumor depth,neck width and tumor width].The correlation between VWI-MRI parameters and the stability,growth and rupture risk of UIA within 5 years was analyzed.Results There were significant differences between stable group and unstable group and between low-risk and high-risk patients with respect to bifurcation aneurysm,morphology,type of reinforcement,grade of wall reinforcement and WEI(P<0.05,P<0.01).The stability,growth within 5 years and rupture risk of UIA were positively correlated with bifurcation aneurysms,morphology,type of reinforcement,grade of wall reinforcement and WEI(P<0.01).Conclusion VWI-MRI can be used to evaluate the stability of UIA,and provide reference for early clinical assessment of aneurysm growth and rupture risk.
