关节镜辅助下自体半腱肌双束解剖重建内侧髌股韧带联合Elmslie-Trillat技术治疗复发性髌骨脱位的疗效分析

摘要 目的 分析关节镜辅助下自体半腱肌双束解剖重建内侧髌股韧带(MPFL)联合胫骨结节内移截骨(Elmslie-Trillat技术)治疗复发性髌骨脱位(RPD)的疗效。方法 回顾性分析21例复发性髌骨脱位患者的临床资料,所有患者均行关节镜辅助下自体半腱肌双束解剖重建MPFL联合Elmslie-Trillat技术治疗。分析患者随访情况,比较患者手术前后Tegner、Kujala、Lysholm评分。结果 所有患者均获有效随访,随访时间1~3年,平均1.5年,术后2个月患者均能正常活动,活动无恐惧感。术后1个月,患者Tegner、Kujala、Lysholm评分分别为(5.45±1.24)、(92.71±3.51)、(91.62±2.77)分,均较术前的(3.06±0.91)、(50.23±3.84)、(54.12±2.90)分明显提高(P<0.05)。结论 关节镜辅助下自体半腱肌双束解剖重建MPFL联合Elmslie-Trillat技术治疗复发性髌骨脱位可有效恢复膝关节解剖结构,改善膝关节功能。 Objective To analyze the efficacy of anatomical double-bundle medial patellofemoral ligament(MPFL)reconstruction with autologous semitendinosus under arthroscopy combined with Elmslie-Trillat technique in the treatment of recurrent patellar dislocation(RPD).Methods The clinical data of 21 patients with recurrent patellar dislocation were retrospectively analyzed.All patients were treated with anatomical doublebundle medial patellofemoral ligament reconstruction with autologous semitendinosus under arthroscopy combined with Elmslie-Trillat technique.The follow-up of patients was analyzed,and the scores of Tegner,Kujala and Lysholm before and after surgery were compared.Results All patients were effectively followed up for 1-3 years,with an average of 1.5 years.2 months after surgery,all patients could exercise normally without fear.1 month after surgery,the scores of Tegner,Kujala and Lysholm were(5.45±1.24),(92.71±3.51)and(91.62±2.77)points,which were significantly higher than(3.06±0.91),(50.23±3.84)and(54.12±2.90)points before surgery(P<0.05).Conclusion Anatomical double-bundle medial patellofemoral ligament reconstruction with autologous semitendinosus under arthroscopy combined with Elmslie-Trillat technique is effective in treating recurrent patellar dislocation,which can effectively restore the anatomical structure of the knee joint and improve the knee joint function.
