改良3D打印导板在肾结石经皮肾镜碎石取石术中的应用

摘要 目的基于改良3D打印导板的设计理念与材质,探讨其在经皮肾镜碎石取石术中的应用效果。方法采用镂空“D”形设计、光敏树脂材料对3D打印导板进行改良。53例经皮肾镜碎石取石术的患者中,采用改良3D打印导板辅助下经皮肾镜碎石取石术(实验组)10例,传统超声引导下经皮肾镜碎石取石术(对照组)43例,对比两组患者的肾镜通路建立时间、手术时间、术中出血、术前术后肌酐和血红蛋白变化幅度及住院时间等临床数据。结果实验组的肾镜通路建立时间、术中出血量、住院时长均少于对照组(P均<0.05)。而手术时间、术后肌酐变化幅度、术后血红蛋白变化幅度等指标,两组间差异均无统计学意义(P均>0.05)。结论改良3D打印导板辅助经皮肾镜碎石取石术可提高手术精准性,减少并发症,降低患者住院花费,值得临床推广应用。 Objective To explore the application efficacy of the improved 3D printing template in percutaneous nephrolithotomy for fragmenting and removing kidney stones,focusing on the design concept and material of the template.Methods The improved 3D printing template was designed with a hollow“D”shape and fabricated using photosensitive resin material.Among 53 patients undergoing percutaneous nephrolithotomy,10 patients were assigned to the experimental group,where the surgery were performed with the assistance of the improved 3D printing template,while 43 patients were assigned to the control group,where the surgeries were guided by traditional ultrasound.Clinical data,including the time for establishing the renal access,surgical duration,intraoperative bleeding,postoperative infection rate,residual stone rate,pre-operative and post-operative creatinine and hemoglobin changes,and length of hospital stay,were compared between the two groups.Results The experimental group exhibited shorter renal scope access time,less intraoperative bleeding,and reduced hospital stay compared to the control group(P all<0.05).However,there was no statistically significant difference in surgical duration,changes in postoperative creatinine and hemoglobin levels,stone-free rate,and postoperative infection rate between the two groups(P all>0.05).Conclusion The improved 3D printing template can enhance surgical precision,reduce complications,and decrease hospital expenses in percutaneous nephrolithotomy procedures.It is worthy of clinical promotion and application.
