结直肠癌永久性造口患者社会疏离感与病耻感、社会支持的相关性

Correlation between social alienation,stigma and social support in colorectal cancer patients with permanent stoma

目的:调查结直肠癌永久性造口患者社会疏离感现状,探讨其影响因素,并分析社会支持、病耻感与社会疏离感的相关性。方法:选取240例结直肠癌永久性造口患者,采用一般资料调查表、社会影响量表、社会支持评定量表和一般疏离感量表对其进行问卷调查。结果:结直肠癌永久性造口患者社会疏离感得分(46.52±6.34)分、社会支持得分(37.64±5.72)分、病耻感得分(68.29±8.34)分;结直肠癌永久性造口患者社会疏离感与社会支持间呈负相关、与病耻感间呈正相关(P<0.05);女性、18~44岁、家庭经济入不敷出、症状困扰明显、抑郁情绪、社会支持程度低、病耻感程度高,其社会疏离感得分高(P<0.05)。结论:建议临床医务人员应重点关注结直肠癌永久性造口患者社会疏离感影响因素,采取针对性措施以降低其社会疏离感。

Objective:To investigate the current status of and factors affecting social detachment in colorectal cancer patients with permanent stoma,and analyze the correlation between social support,stigma and social alimentation.Methods:240 colorectal cancer patients with permanent stoma were included,and undergone questionnaire survey using general information scale,Social Impact Scale,Social Support Rating Scale and General Alienation Scale.Results:The scores were(46.52±6.34),(37.64±5.72)and(68.29±8.34)points,respectively on social alienation,social support and stigma for the patients with permanent colostomy,and the patients′social alienation was negatively correlated with social support,yet positively with stigma(P<0.05).The scoring on social alienation was higher in males,patients aged 18-44 years,those with poor family income,obvious symptoms,depressed mood,lower social support and higher stigma(P<0.05).Conclusion:Clinical medical staff are supposed to pay more attention to the factors affecting social alienation,and take targeted measures to reduce the detachment in colorectal cancer patients with permanent stoma.
