便携式呼吸机结合纳洛酮对急性呼吸衰竭患者院前急救的应用效果

摘要 目的分析便携式呼吸机结合纳洛酮对急性呼吸衰竭患者院前急救的应用效果。方法研究纳入唐山市妇幼保健院2020年9月—2021年9月收治行纳洛酮+常规辅助通气治疗院前急救急性呼吸衰竭患者33例为对照组,纳入2022年10月—2023年11月行便携式呼吸机+纳洛酮治疗院前急救急性呼吸衰竭患者33例试验组,比较两组间院前急救时间、血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧指数(OI)以及血氧饱和度(SaO_(2))]、生命体征[心率(HR)及呼吸频率(RR)]及抢救成功率。结果试验组院前急救时间(16.63±3.06)min短于对照组(21.63±2.93)min(P<0.05);试验组与对照组的急救前、急救10 min后血气指标(PaO_(2)、PaCO_(2)、OI、SaO_(2))组间差异无统计学意义(P>0.05),试验组转运前PaO_(2)、OI、SaO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05);试验组与对照组急救前生命体征指标(HR、RR)差异无统计学意义(P>0.05),试验组急救10 min后、转运前HR、RR低于对照组(P<0.05);试验组院前抢救成功率同对照组组间无统计学差异(100.00%vs.96.67%)(P>0.05)。结论便携式呼吸机结合纳洛酮能更好地改善急性呼吸衰竭患者院前急救中血气指标及生命体征,缩短院前急救时间,提升院前急救成功率。 Objective To analyze the application effect of portable ventilator combined with naloxone in pre-hospital emergency treatment for patients with acute respiratory failure.Methods A total of 33 patients with acute respiratory failure who received routine assisted ventilation with naloxone in pre-hospital emergency care at Tangshan Women and Children's Hospital from September 2020 to September 2021 were included as the control group.Another 33 patients with acute respiratory failure who received portable ventilators combined with naloxone in pre-hospital emergency care from October 2022 to November 2023 were included as the experimental group.The pre-hospital emergency treatment time between groups under different pre-hospital emergency treatment methods was compared Blood gas indicators[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygen index(OI)and blood oxygen saturation(SaO_(2))],vital signs[heart rate(HR)and respiratory rate(RR)],and success rate of rescue.Results Compared with the control group(21.63±2.93)minutes,the study group had a shorter pre-hospital emergency response time(16.63±3.06)minutes(P<0.05);there were no statistically significant differences in blood gas analysis parameters(PaO_(2),PaCO_(2),OI,SaO_(2))between the experimental group and the control group before emergency care and 10 minutes after emergency care(P>0.05).Compared with the control group before transport,the study group had higher PaO_(2),OI,SaO_(2)and lower PaCO_(2)(P<0.05).There was no statistical value in HH and HR between groups compared with the control group before first aid(P>0.05),but the HH and HR in the study group were lower than those in the control group 10 min before first aid and before transport(P<0.05).There was no statistical value in the success rate of rescue between groups(100.00%vs.96.67%)(P>0.05).Conclusion The combination of portable ventilators with naloxone can effectively improve blood gas analysis parameters and vital signs in patients with acute respiratory failure during pre-hospital emergency care,reduce pre-hospital emergency care time,and enhance the success rate of pre-hospital emergency care.
