基于黄元御土枢四象一气周流理论论治小儿肠系膜淋巴结炎

Treatment of Pediatric Mesenteric Lymphadenitis Based on Huang Yuanyu’s Theory of Zangfu Model with the Spleen and Stomach as the Pivot and General Circulation of Unitary Qi

小儿肠系膜淋巴结炎是儿科临床常见病,常继发于外感病或肠道炎症后,其典型症状表现为迸发腹痛,反复发作,病情缠绵。中医药作为一种综合疗法对此病效果确切。黄元御乃清代著名医家,文章基于其“土枢四象,一气周流”理论,从气机升降的角度讨论小儿肠系膜淋巴结炎的论治,认为此病病机主要与脾虚湿困、肝木郁遏、肾阳亏虚有关,治疗上应从“培土祛湿、固护阳气”“疏肝利胆、调理气机”“补肾助阳、以温寒泉”等方面着手,以恢复机体“一气”之周流。

Pediatric mesenteric lymphadenitis is a common clinical disease in pediatrics.It is often secondary to exogenous diseases or intestinal inflammation.Its typical symptoms are sudden abdominal pain,repeated attacks and lingering illness.Traditional Chinese medicine,as a comprehensive treatment,has a definite effect on the treatment of this disease.Huang Yuanyu is a famous doctor in the Qing Dynasty.Based on his theory of“zangfu model with the spleen and stomach as the pivot and general circulation of unitary qi”,this paper discussed the treatment of mesenteric lymphadenitis in children from the angle of ascending and descending of qi.He be-lieves that the pathogenesis of this disease is mainly related to the spleen deficiency and the stagnation of dampness,the stagnation of liver-wood,and the deficiency of kidney-yang.The treatment should start from cultivating soil to remove dampness,consolidating and protecting yang qi,soothing the liver and gallbladder,regulating qi,tonifying the kidney and helping yang,and warming the cold spring to restore the circulation of“unitary qi”of the body.
