2型糖尿病患者护理中Triangle分层分级管理联合LEARNS模式的应用效果

Application Effect of Triangle Stratified Hierarchical Management Com⁃bined with LEARNS Model in Nursing Care of Patients with Type 2 Diabe⁃tes Mellitus

摘要 目的探究2型糖尿病(Type 2 Diabetes Mellitus,T2DM)患者护理中Triangle分层分级管理联合LEARNS模式的应用效果。方法选取武威市天祝藏族自治县人民医院于2021年2月—2023年2月收治的280例T2DM患者为研究对象,以随机信封法分为常规组(n=140,常规护理)、观察组(n=140,常规组基础上行Triangle分层分级管理联合LEARNS模式)。观察并对比护理前后组间、组内各指标之间的差异。结果干预前组间血糖水平、自我管理能力评分对比,差异无统计学意义(P均>0.05);组间干预6个月后血糖水平对比,观察组更低,差异有统计学意义(P<0.05);组间干预6个月后自我管理能力评分对比,观察组(85.41±3.12)分高于常规组(84.23±3.19)分,差异有统计学意义(t=3.129,P<0.05)。结论Triangle分层分级管理联合LEARNS模式护理T2DM,可改善血糖水平及提高患者自我管理能力。 Objective To investigate the application effect of Triangle stratified hierarchical management combined with LEARNS model in the care of Type 2 Diabetes Mellitus(T2DM)patients.Methods A total of 280 cases of T2DM patients admitted to Tianzhu Tibetan Autonomous County People's Hospital from February 2021 to February 2023 were selected as study objects and divided into conventional group(n=140,conventional care)and observation group(n=140,Triangle stratified hierarchical management combined with LEARNS mode on the basis of the conventional group)by the random envelope method.Observe and compare the differences between the groups before and after the care and between the indicators within the groups.Results Comparison of blood glucose level and self-management ability score between groups before intervention,and the differences were not significantly significant(both P>0.05).Comparison of blood glucose levels between groups after 6 months of intervention,the observation group was lower,and the difference was significantly significant(P<0.05).Comparison of self-management ability score after 6 months of intervention between groups,observation group(85.41±3.12)points was higher than the conventional group(84.23±3.19)points,and the difference was significantly significant(t=3.129,P<0.05).Conclusion Triangle stratified hierarchical management combined with LEARNS model of care for T2DM can improve blood glucose levels and pa-tients'self-management ability.
