液基薄层细胞学联合第二代杂交捕获法检测高危型人乳头瘤病毒在宫颈癌筛查中的应用

摘要 目的探析液基薄层细胞学(TCT)联合第二代杂交捕获法检测高危型人乳头瘤病毒(HPV-DNA)诊断宫颈癌的效能。方法从2020—2022年我院收治的疑似宫颈癌患者中抽取116例开展研究,所有患者均接受TCT、第二代杂交捕获法检测高危型HPV-DNA检测以及病理活检,以病理活检诊断结果为金标准,分析TCT、第二代杂交捕获法检测高危型HPV-DNA 2种诊断方法单一和联合诊断的效能。结果TCT、第二代杂交捕获法检测高危型HPV-DNA检测效能相近(P>0.05);2种联合诊断与单一诊断相比,联合诊断灵敏度、阴性预测值和准确率较高(P<0.05),漏诊率和误诊率较低(P<0.05);TCT、第二代杂交捕获法检测高危型HPV-DNA单一诊断和联合诊断的特异度和阳性预测值相近(P>0.05)。结论TCT、第二代杂交捕获法检测高危型HPV-DNA检测在宫颈癌筛查均具有良好诊断效能,两者联合诊断可进一步提高诊断的精准度和灵敏度。 Objective To explore the efficacy of liquid-based thin-layer cytology(TCT)combined with second-generation hybridization capture assay for the detection of high-risk human papillomavirus(HPV-DNA)test for the diagnosis of cervical cancer.Methods From January 2020 to December 2022 one hundred and sixteen suspected cervical cancer patients admitted to our hospital were selected for the study.All the patients were subjected to TCT,second-generation hybridization capture method for detecting high-risk HPV-DNA detection as well as pathological biopsy,and the efficacy of the two diagnostic methods of TCT and second-generation hybridization capture method for detecting high-risk HPV-DNA detection,both singly and in combination,was analyzed with the diagnostic results of the pathological biopsy as the gold standard.Results The detection efficacy of high-risk HPV-DNA by TCT and second-generation hybridization capture method was similar(P>0.05);compared with the single diagnosis of the two diagnostic methods,the sensitivity,negative predictive value and accuracy of the combined diagnosis were higher(P<0.05),and the rate of missed diagnosis and misdiagnosis were lower(P<0.05).The specificity and positive predictive value of the single diagnosis and combined diagnosis of high-risk HPV-DNA by TCT and second-generation hybridization capture method were similar(P>0.05).Conclusion TCT and second-generation hybridization capture assay for high-risk HPV-DNA detection have good diagnostic efficacy in cervical cancer screening,and their combined diagnosis can further improve diagnostic precision and sensitivity.
