眶隔脂肪释放重置术治疗重度泪沟畸形患者的美容效果

Cosmetic results of orbital septal fat release reset in patients with severe lacrimal groovedeformity

目的研究眶隔脂肪释放重置术对于治疗重度泪沟畸形患者的美容效果。方法选择2021年10月-2023年6月,于本院接受治疗的136例重度泪沟畸形患者,均接受眶隔脂肪释放重置术,术前、术后通过Barton分级、泪沟畸形评定量表评估患者的美容效果,术后对患者进行为期6个月的随访。记录患者并发症、复发情况,收集患者对美容效果的满意情况。结果术后6个月Barton分级0级87(63.97%)例,Ⅰ级29(21.32%)例,Ⅱ级20(14.71%)例,总改善率100%;术后6个月,TTRS各项指标及总评分均较术前显著降低(P<0.05);所有切口均Ⅰ期愈合,术后6个月内出现轮廓不规则或移位2例(1.47%),瘀斑10(7.35%)例,球结膜出血5(3.68%例),局部结节4(2.94%)例,总发生21(15.44%)例;术后患者评价非常满意64例(47.06%),满意33例(24.26%),一般30(22.06%)例,不满意9(6.62%)例,总满意率93.38%(127/136)。结论眶隔脂肪释放重置术是一种长效、稳定、安全改善患者泪沟畸形的方法,对于重度泪沟畸形患者的美容有良好的效果。

Objective To study the cosmetic effect of orbital septal fat release reset in the treatment of patients with severe lacrimal groove deformity.Methods From October 2021 to March 2023,136 patients with severe lacrimal groove deformity were treated in our hospital,all underwent orbital septal fat release reset,the cosmetic effect was evaluated by Barton grade and tear rating scale,and the patients were followed up for 6 months after surgery.Patient complications and recurrence were recorded and patients'satisfaction with cosmetic results was collected.Results 87(63.97%)6 months after surgery,Grade I 29(21.32%),Grade 20(14.71%),Total improvement rate of 100%;Six months after the surgery,All indicators and total score of TTRS were significantly lower compared with preoperative values(P<0.05);All incisions were healed in stage I,Two cases of contour irregularities or shift(1.47%),10 cases of ecchymosis(7.35%),Bulbar conjunctival hemorrhage 5(3.68%cases),Local nodules 4(2.94%),Total occurrence of 21(15.44%)cases;The postoperative patient evaluation was very satisfactory,and 64 patients(47.06%),33 cases(24.26%),General 30(22.06%)cases,Dissatisfied with 9(6.62%),The overall satisfaction rate was 93.38%(127/136).Conclusion Orbitital septal fat release reset is a long-term,stable and safe method to improve lacrimal groove deformity,which has good cosmetic effect for patients with severe lacrimal groove deformity.
