摘要
目的本研究旨在了解新生儿粪便分离的多重耐药准肺炎克雷伯菌的基因组和耐药相关基因特征。方法使用E-test抗生素敏感性检测和全基因组测序技术和生物信息分析方法。结果该菌株对阿莫西林/克拉维酸、头孢他啶、头孢唑啉、头孢呋辛、头孢曲松、头孢哌酮、美罗培南等临床常用抗生素耐药。全基因组测序提示该菌株携带两个质粒,与以往报道的IncFⅡ型和IncX3型质粒高度相似。并且,两个质粒携带重要的耐药基因,分别为bla TEM-1D、bla CTX-M-15、bla NDM-1和bla SHV-12。同时染色体DNA上也携带一些重要耐药相关基因,如bla OKP-B-2等。这些耐药基因能够解释我们检测到的耐药表型的发生,并说明这些耐药性可能随着质粒具有一定的传播风险。结论准肺炎克雷伯菌不同于肺炎克雷伯菌,其耐药特性和基因组具有一定特征。并且,由于新生儿可携带,且具有多重耐药性,应该受到进一步的广泛关注。
Objective In this study,we aimed to study the complete genome and multidrug resistance characteristics of Klebsiella quasipneumoniaen subsp.similipneumoniae isolated from neonatal feces.Methods E-test antibiotic sensitivity detection,whole genome sequencing technology and bioinformation analysis were used.Results This strain possessed multidrug resistance and was resistant to the antibiotics commonly used in clinic,including amoxicillin clavulanate,ceftazidime,cefazolin,cefuroxime,ceftriazmecefoperazone,and meropenem.Whole genome sequencing suggests that this strain carries two plasmids,which are highly similar to the previously reported plasmids IncFII and IncX3.Moreover,the two plasmids carry important carbapenem resistance genes,namely bla TEM-1D,bla NDM-1,bla CTX-M-15 and bla SHV-12.At the same time,the chromosome DNA also carries some important drug resistance genes,such as bla OKP-B-2.These resistance genes can explain the occurrence of the resistance phenotypes and suggest that these resistance genes have a certain risk of transmission along with the plasmid.Conclusion This study suggests that we should distinguish between Klebsiella quasipneumoniae and Klebsiella pneumoniae and focus on the infection and drug resistance in the future.
