^(18)F-FDG PET CT联合CA125在早期卵巢癌中诊断价值的Meta分析

摘要 目的:系统评价^(18)F-FDG PET/CT联合CA125在早期卵巢癌中的诊断价值。方法:检索2010年1月—2023年8月英文数据库及中文数据库中^(18)F-FDG PET/CT联合CA125在早期卵巢癌中诊断价值的诊断性实验研究,使用Review Mansger5.4软件中的QUADAS-2质量评价工具对纳入的研究进行文献质量评估,使用Meta-Disc1.4软件合并灵敏度、特异度、阳性似然比、阴性似然比、诊断比值比。使用STATA14.0软件对所纳入的研究进行发表偏倚检验。结果:共纳入了4项研究,260名早期卵巢癌患者,^(18)F-FDG PET/CT联合CA125诊断早期卵巢癌的合并灵敏度为0.91(0.86,0.95),合并特异度为0.95(0.88,0.99),合并阳性似然比为14.66(6.00,35.81),合并阴性似然比为0.10(0.07,0.16),合并诊断比值比为166.79(55.07,505.19),AUC=0.9737,研究结果稳定,不存在发表偏倚。结论:^(18)F-FDG PET/CT联合CA125对早期卵巢癌的诊断效能较好,在早期卵巢癌诊断中值得临床推广应用。 Objective Evaluation of the diagnostic value of ^(18)F-FDG PET/CT combined with CA125 in early-stage ovarian cancer.Methods A diagnostic experimental study on the diagnostic value of ^(18)F-FDG PET/CT combined with CA125 in the early diagnosis of ovarian cancer was conducted by retrieving English and Chinese databases from January 2010 to August 2023.The included studies were assessed for their quality using the QUADAS-2 tool in Review Manager 5.4 software.Meta-Disc 1.4 software was used to combine sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,and diagnostic odds ratio.Publication bias was assessed using STATA 14.0 software.Results Four studies involving 260 patients with early ovarian cancer were included.The combined sensitivity and specificity of ^(18)F-FDG PET/CT combined with CA125 for diagnosing early ovarian cancer were 0.91(0.86-0.95)and 0.95(0.88-0.99),respectively.The combined positive likelihood ratio was 14.66(6.00-35.81),and the combined negative likelihood ratio was 0.10(0.07-0.16).The combined diagnostic odds ratio was 166.79(55.07-505.19).The AUC was 0.9737,indicating a stable and unbiased research outcome.Conclusion The combination of ^(18)F-FDG PET/CT and CA125 demonstrates better diagnostic efficacy for early-stage ovarian cancer,and is worthy of clinical promotion and application in the diagnosis of early-stage ovarian cancer.
