重症呼吸道感染行纤维支气管镜治疗的有效性

Effectiveness of fiberoptic bronchoscopy in the treatment of severe respiratory tract infections

摘要 目的研究纤维支气管镜治疗重症呼吸道感染的有效性,为提升临床治疗效果提供参考。方法选取2019年10月—2021年10月高唐县人民医院收治的重症呼吸道感染患者82例进行研究,采用随机抽签法分为观察组和对照组各41例。对照组采用常规对症治疗,包括吸氧、吸痰,机械通气,抗感染等治疗,应用吸痰器定时吸痰,用糖皮质激素控制病情,观察组在对照组的基础上采用纤维支气管镜治疗,麻醉后应用纤维支气管镜通过中心负压吸痰治疗,并反复用生理盐水进行冲洗。比较两组患者每分钟通气量、治疗总有效率。结果治疗后,观察组每分钟通气量为(8.94±1.42)L/min,明显高于对照组的(7.03±1.38)L/min,差异有统计学意义(t=6.176,P<0.05)。观察组治疗总有效率为95.12%,高于对照组的75.61%,差异有统计学意义(χ^(2)=6.247,P<0.05)。结论重症呼吸道感染行纤维支气管镜治疗能有效提高患者的治疗总有效率,改善患者紫绀、吸痰等症状,提高通气量,值得临床推广应用。 Objective To study the effectiveness of fiberoptic bronchoscopy in the treatment of severe respiratory tract infections,in order to provide reference for improving clinical treatment effectiveness.Methods A total of 82 patients with severe respiratory tract infections admitted to Gaotang County People's Hospital from October 2019 to October 2021 were selected for the study.They were divided into observation group and control group by random drawing method,with 41 cases in each group.The control group received routine symptomatic treatment,including oxygen inhalation,sputum suction,mechanical ventilation,anti infection treatment,etc.The sputum was aspirated at regular intervals using a suction device,and the condition was controlled with glucocorticoids.The observation group received fiberoptic bronchoscopy treatment on the basis of the control group,followed by central negative pressure suction of fiberoptic bronchoscopy after anesthesia,and repeated rinsing with physiological saline.The ventilation volume per minute and the total effective rate of treatment between two groups of patients were compared.Results After treatment,the observation group had a ventilation volume per minute of(8.94±1.42)L/min,which was significantly higher than the control group's(7.03±1.38)L/min,and the difference was statistically significant(t=6.176,P<0.05).The total effective rate of the observation group was 95.12%,which was higher than the control group's 75.61%,and the difference was statistically significant(χ^(2)=6.247,P<0.05).Conclusion The use of fiberoptic bronchoscopy for the treatment of severe respiratory tract infections could effectively improve the total effective rate of patients,improve symptoms such as cyanosis and sputum aspiration,and increase ventilation volume,which was worthy of clinical promotion and application.
