改良衰弱指数与睡眠质量预测老年患者术后谵妄发生的价值

The value of improved frailty index and sleep quality in predicting postoperative delirium in elderly patients

目的探索改良衰弱指数(Modified frailty index,mFI)与睡眠质量(Pittsburgh Sleep Quality Index,PSQI)评分预测老年患者术后谵妄发生的价值。方法回顾性分析322例老年手术患者均在2020年8月至2023年8月期间收治,根据患者术后有无谵妄分为两组,即发生组(n=28),未发生组(n=294),比较两组一般资料,经二元Logistic回归分析影响患者术后发生谵妄的独立因素,同时,再经ROC曲线分析mFI评分、PSQI评分预测价值。结果经二元Logistic回归分析,年龄>75岁、PSQI评分、m FI评分是影响老年患者术后发生谵妄的独立因素(P<0.05)。经ROC曲线分析,PSQI评分、mFI评分预测老年患者术后谵妄发生的AUC分别为0.926、0.827。结论PSQI评分、m FI评分能够预测老年患者术后谵妄发生情况,利用价值较高。

Objective Exploring the value of improved frailty index(mFI)and sleep quality index(PSQI)scores in predicting postoperative delirium in elderly patients.Methods A retrospective analysis of 322 elderly surgical patients was conducted between August 2020 and August 2023.They were divided into two groups based on the presence or absence of postoperative delirium,namely the occurrence group(n=28)and the non-occurrence group(n=294).The general data of the two groups were compared,and independent factors affecting postoperative delirium were analyzed through binary logistic regression.At the same time,the predictive value of mFI score and PSQI score was analyzed through ROC curve analysis.Results Affer binary logistic regression analysis,age>75 years old,PSQI score,and mFI score were independent factors affecting postoperative delirium in elderly patients(P<0.05).According to ROC curve analysis,the AUC predicted by PSQI score and mFI score for postoperative delirium in elderly patients is 0.926 and 0.827,respectively.Conclusion PSQI score and mFI score can predict the occurrence of postoperative delirium in elderly patients,and have high utilization value.
