老年口腔颌面部疼痛患者负性情绪状况及其影响因素

Negative emotional status in oral and maxillofacial pain elderly patients and its risk factors

目的探究老年口腔颌面部疼痛患者负性情绪状况及其影响因素。方法选取2020年5月~2023年5月期间来我院就诊的老年患者379例,评估其负性情绪(焦虑、抑郁)发生情况,并采用二元logistic回归分析口腔颌面部疼痛对负性情绪的影响。结果本研究共纳入379名老年患者,其中患有口腔颌面部疼痛的178例(46.97%),67人(17.68%)存在抑郁,18人(4.75%)存在焦虑。不同人口学特征下焦虑抑郁症状对比分析,调查结果显示,不同婚姻状况、睡眠时长、吸烟及口腔颌面部疼痛患者焦虑症状检出率具有明显差异(P<0.05);不同性别、年龄、文化程度、养老金状况、婚姻状况、睡眠时长、饮酒、吸烟、慢性病及口腔颌面部疼痛患者抑郁症状检出率具有明显差异(P<0.05)。采用二元Logistic回归分析,结果显示,控制人口学特征等因素下,口腔颌面部疼痛与焦虑症状呈正相关(OR:0.058,95%CI:0.058-0.023),与抑郁症状呈正相关(OR:0.058,95%CI:0.058-0.023)。结论老年口腔颌面部疼痛与焦虑抑郁情绪有关,临床工作中,医护人员、家庭及社会应当时刻关注老人口腔颌面部疼痛情况,以便于及时对症处理,改善老人心理健康及生活质量。

Objective To explore the negative emotional status in oral and maxillofacial pain elderly patients and its risk factors.Methods 379 elderly patients who came to our hospital from May 2020 to May 2023 were selected to analyze the incidence of negative emotions(anxiety and depression),and the influence of oral and maxillofacial pain on negative emotions was analyzed by binary logistic regression.Results A total of 379 elderly patients were included in this study,of which 178 patients(46.97%)had oral and maxillofacial pain,67 patients(17.68%)had depression,and 18 patients(4.75%)had anxiety.The results showed that there were significant differences in the detection rate of anxiety symptoms in patients with different marital status,sleep duration,smoking and oral and maxillofacial pain(P<0.05).There were significant differences in the detection rate of depressive symptoms in patients with different gender,age,education,pension status,marital status,sleep duration,drinking,smoking,chronic diseases and oral and maxillofacial pain(P<0.05).Binary Logistic regression analysis showed that oral and maxillofacial pain was positively correlated with anxiety symptoms(OR:0.058,95%CI:0.058-0.023)and depression symptoms(OR:0.058,95%CI:0.058-0.023)when controlling demographic characteristics and other factors.Conclusion The elderly oral and maxillofacial pain is related to anxiety and depression.In clinical work,medical staff,family and society should always pay attention to the elderly oral and maxillofacial pain,in order to timely symptomatic treatment and improve the mental health and quality of life of the elderly.
