摘要
目的探讨鼻部、眉间、额部注射透明质酸钠凝胶导致血管栓塞的治疗方法及效果。方法2021年3月至2023年2月,天津市中西医结合医院整形修复外科及天津市河北区时光整形美容门诊部收治注射透明质酸钠凝胶后血管栓塞女性患者9例,年龄19~44(32.6±2.3)岁,均给予急诊手术多点穿刺减压、局部低分子肝素及透明质酸溶解酶皮下注射,并改善微循环、抗感染、局部创面换药及抗瘢痕等相应治疗。结果9例患者经过综合治疗3~10 d后,皮肤症状均逐渐恢复正常,局部无组织坏死,随访6个月未遗留明显瘢痕,效果满意。结论注射透明质酸钠凝胶导致血管栓塞患者及时给予多点穿刺,联合低分子肝素及透明质酸溶解酶皮下注射等综合治疗后,能取得较好的效果。
Objective To investigate the anatomic mechanism and treatment of vascular embolism caused by hyaluronic acid injection in nasal,interglabella and frontal parts.Methods A total of 9 patients with post-injection vascular embolism admitted to Tianjin Hospital of Integrated Traditional Chinese and Western Medicine and the Time Plastic and Aesthetic Clinic of Hebei District of Tianjin from March 2021 to February 2023 were selected.aged from 19 to 44(32.6±2.3)years all patients received emergency surgery with multi-point puncture decompression and subcutaneous injection of local low molecular weight heparin and hyaluronic acid lytic enzyme.And improving the microcirculation,anti-infection,local wound dressing and antiscar related treatment were carried out,the therapeutic effect and patients′satisfaction were statistically analyzed.Results The symptoms of 9 patients gradually returned to normal after 3 to 10 days by the above treatment,and there was no tissue necrosis and scar left after 6 months follow-up,and the effects were satisfactory.Conclusions For patients with vascular embolism caused by hyaluronic acid injection,multi-point puncture combined with subcutaneous injection of low molecular weight heparin and hyaluronic acid lyolytic enzyme can achieve satisfactory results.
