基于岗位胜任力的N1级护士培训体系构建及实施效果

摘要 目的:基于岗位胜任力构建适用于综合医院的N1级护士规范化层级培训体系,并探讨其在临床实施中的应用效果,为提升N1级护士岗位胜任力、提高综合能力提供参考依据。方法:通过文献检索N1级护士培训相关词条,初步拟定N1级护士培训体系框架,采用德菲尔专家咨询法进行2轮专家函询,最终构建N1级护士层级培训体系。采用整群抽样法,选取上海市第一人民医院酒泉医院2022年1月—2023年12月的全体在岗N1级护士作为研究对象,将2022年1—12月培训体系实施前的198名护士作为对照组,将2023年1—12月培训体系实施后的207名护士作为研究组,比较两组护士岗位胜任力变化。结果:经过2轮专家函询,专家积极系数均为100%;权威系数2.935、2.886;专家协调系数第1轮分别为0.388、0.336、0.190;第2轮分别为0.117、0.270、0.137,最终形成N1级护士培训一级指标5项、二级指标10项、三级指标54项,完成了207名护士的培训工作。研究组的护士岗位胜任力得分高于对照组,差异有统计学意义(P<0.05)。结论:N1级护士培训体系的构建具有较高的科学性,能提升护士岗位胜任力,改善临床护理质量,对综合医院N1级护士的培养、考核、任用具有参考价值。 Objective Based on the competency framework,this study aims to construct a standardized hierarchical training system for N1-level nurses in comprehensive hospitals and explore its application effectiveness in clinical practice,providing reference for enhancing the competency and comprehensive abilities of N1-level nurses.Methods Relevant literature on N1-level nurse training was searched to preliminarily establish the framework of the training system.The Delphi expert consultation method was used in 2 rounds of expert consultations to construct the hierarchical training system for N1-level nurses.Using cluster sampling,all N1-level nurses on duty in Jiuquan Hospital of Shanghai First People's Hospital from January 2022 to December 2023 were selected as the research subjects.The 198 nurses before the implementation of the training system from January to December 2022 were the control group,and the 207 nurses after the implementation of the training system from January to December 2023 were the study group.The changes of nurses'post competency were compared between the two groups.Results After 2 rounds of expert consultations,the positive coefficient of the experts was 100%;The authority coefficient was 2.935 and 2.886;The expert coordination coefficient in the first round was 0.388,0.336,0.190,and in the second round was 0.117,0.270,0.137.Finally,a total of 5 primary indicators,10 secondary indicators,and 54 tertiary indicators were formed for N1-level nurse training,and training was completed for 207 nurses.The competency score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusions The construction of the N1-level nurse training system is scientifically rigorous and can enhance nurses'competency,improve clinical nursing quality,and provide reference value for the training,assessment,and appointment of N1-level nurses in comprehensive hospitals.
