门诊华法林使用情况调查及用药分析

Investigation and analysis of warfarin use in outpatient department

目的:了解我院门诊华法林的使用情况,探讨用药的合理性。方法:对郑州人民医院2022年1月-6月门诊患者使用华法林的处方进行调查,统计分析患者的基本资料、用药科室、临床诊断、用法用量及合并用药情况。结果:共抽取处方400张,男207例,女193例,平均年龄67.89±13.13岁,涉及19个科室,23种临床诊断和14种用法用量。联合用药方面,共有化学药20种,中成药11种可影响华法林的抗凝作用。结论:我院华法林应用尚不规范,需规范华法林处方的适应证,用法用量,加强监测国际标准化比值,关注华法林与其他药物之间的相互作用,临床药师应发挥专业特长,加强华法林的用药教育及监护,提高华法林抗凝患者治疗的安全性和有效性。

Objective:To investigate the use of warfarin in outpatients of our hospital,and to explore the rationality of drug use.Method:The prescriptions of warfarin in outpatients of Zhengzhou People's Hospital from January to June 2022 were investigated.The basic data,administration departments,clinical diagnosis,usage,dosage and drug combination of warfarin were statistically analyzed.Result:A total of 400 prescriptions were collected,207 males and 193 females,with an average age of 67.89±13.13 years old,involving 19 departments,23 kinds of clinical diagnosis and 14 kinds of usage and dosage.In combination,there were 20 kinds of chemical drugs,and 11 kinds of Chinese patent medicine could affect the anticoagulant effect of warfarin.Conclusion:The application of warfarin in our hospital is still not standardized.It is necessary to standardize the indications,usage and dosage of warfarin prescription,strengthen the monitoring of international normalized ratio,pay attention to the interaction between warfarin and other drugs.Clinical pharmacists should give full play to their professional expertise,strengthen medication education and monitoring of warfarin,and improve the safety and effectiveness of warfarin anticoagulant treatment.
