中药坐浴联合高锰酸钾坐浴对产后会阴切口疼痛及伤口愈合的影响

摘要 目的观察中药坐浴联合高锰酸钾坐浴对产后会阴切口疼痛及伤口愈合的影响。方法将126例会阴侧切产妇按照随机数字表法分为2组,对照组63例予高锰酸钾坐浴治疗,观察组63例在对照组治疗基础上予中药坐浴。2组均治疗2周。比较2组治疗前和治疗1、2周后疼痛视觉模拟评分(VAS)及炎症相关因子[白细胞介素1(IL-1)、肿瘤坏死因子(TNF-α)、C反应蛋白(CRP)]水平,比较2组切口愈合率及切口愈合时间,记录2组治疗期间发生的不良反应。结果2组治疗1、2周后疼痛VAS均较本组治疗前降低(P<0.05),且观察组治疗1、2周后均低于对照组同期(P<0.05)。2组切口愈合率比较差异无统计学意义(P>0.05)。观察组切口愈合时间短于对照组(P<0.05)。2组治疗后IL-1、TNF-α、CRP水平均较本组治疗前降低(P<0.05),且观察组治疗后均低于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论中药坐浴联合高锰酸钾坐浴治疗产后会阴切口可明显降低患者切口疼痛程度,缩短切口愈合时间,改善机体炎症反应,不良反应较少。 Objective To explore the effect of sitz bath of self-made Chinese herbal combined with potassium permanganate on postpartum perineal incision pain and healing.Methods A total of 126 parturients underwent lateral episiotomy(LE)were randomly assigned into observation group(n=63)and control group(n=63).All patients were managed by potassium permanganate sitz bath,and those in observation group were additionally treated with self-made Chinese herbal sitz bath.Treated for 2 weeks,the aim was to compare visual analogue scale(VAS),inflammation-related factors(interleukin 1[IL-1],tumor necrosis factor alpha[TNF-α],C-reactive protein[CRP]),improvement of incision pain,wound healing rate,wound healing time.The adverse reactions were recorded.Results VAS score on weeks 1 and 2 in the both groups was significantly decreased(P<0.05),which decreased notably in the observation group compared with the control group(P<0.05).None of the wound healing rate differed significantly between groups(P>0.05).The wound healing time was significantly shorter in the observation group than in the control group(P<0.05).After treatment,IL-1,TNF-α,CRP in the both groups were significantly decreased(P<0.05),which in the observation group were more prevalent than those of the control group(P<0.05).The incidence of adverse reactions was significantly decreased in the observation group relevant to the control group(P<0.05).Conclusion The sitz bath of self-made Chinese herbal combined with potassium permanganate for postpartum perineal incision can significantly reduce pain,shorten the wound healing time,improve inflammatory response,and with less adverse reactions.
