补肾健脾调冲汤加减对多囊卵巢综合征不孕症患者的影响研究

The Effects of Modified Bushen Jianpi Tiaochong Decoction on Patients with Polycystic Ovary Syndrome (PCOS) and Infertility

目的分析多囊卵巢综合征(PCOS)合并不孕症患者取补肾健脾调冲汤加减治疗的实际疗效及对患者子宫内膜厚度、性激素水平的影响。方法将郑州大学附属洛阳中心医院2019年1月至2022年7月PCOS合并不孕症患者(78例)纳入作为此次研究样本,按照随机单双数抽签法分组(单数者入对照组、双数者入研究组),两组各39例。对照组采取氯米芬治疗,研究组在对照组用药基础上增加补肾健脾调冲汤加减。对比两组间治疗有效率、不良反应;对比两组治疗前、治疗后子宫内膜厚度、促黄体生成素(LH)、睾酮(T激素)、症候评分。结果研究组治疗有效率(89.74%)高于对照组(71.79%),差异有统计学意义(P<0.05);治疗前两组子宫内膜厚度、LH、T激素水平对比差异无统计学意义(P>0.05),治疗后两组子宫内膜厚度增加,LH、T激素水平降低,且研究组子宫内膜厚度高于对照组,LH、T激素水平均于对照组,差异有统计学意义(P<0.05);治疗前两组症候评分对比差异无统计学差异(P>0.05),治疗后两组月经紊乱、腰膝酸软、头晕耳鸣评分较治疗前明显降低,研究组评估分值低于对照组,差异有统计学意义(P<0.05);研究组用药期间不良反应发生率(10.26%)与对照组(7.69%)比较,差异无统计学意义(P>0.05)。结论补肾健脾调冲汤加减治疗PCOS合并不孕症疗效确切,可增加子宫内膜厚度,有利于控制性激素水平且可保障安全性,值得应用。

Objective To analyze the practical efficacy of modified Bushen Jianpi Tiaochong decoction on patients with polycystic ovary syndrome(PCOS)complicated with infertility,and its effects on endometrial thickness and sex hormone levels in patients.Methods A total of 78 patients with PCOS complicated with infertility admitted to Luoyang Central Hospital Affiliated to Zhengzhou University from January 2019 to July 2022 were included as the study sample.Patients were divided into two groups using a random odd-even number drawing method(odd-numbered patients were assigned to the control group and even-numbered patients to the study group),with 39 patients in each group.The control group received clomiphene treatment,while the study group received modified Bushen Jianpi Tiaochong decoction in addition to the medication used in the control group.The efficacy rate and adverse reactions were compared between the two groups.The endometrial thickness,luteinizing hormone(LH),testosterone(T)levels,and symptom scores were compared before and after treatment between the two groups.Results The efficacy rate in the study group (89.74%) was higher than that in the control group (71.79%), and the difference was statistically significant (P < 0.05). Before treatment, there was no statistically significant difference in endometrial thickness, LH, and T levels between the two groups (P > 0.05). After treatment, both groups showed an increase in endometrial thickness and a decrease in LH and T levels. The endometrial thickness in the study group was higher than that in the control group, and the LH and T levels were lower than those in the control group, with statistically significant differences (P < 0.05). Before treatment, there was no statistically significant difference in symptom scores between the two groups (P > 0.05). After treatment, the scores for menstrual disorders, lumbago and weakness of the knees, dizziness, and tinnitus in both groups were significantly reduced compared to before treatment. The evaluation scores in the study group were lower than those in the control group, with statistically significant differences (P < 0.05). The incidence of adverse reactions during the medication period was 10.26% in the study group and 7.69% in the control group, with no statistically significant difference (P > 0.05).Conclusion Modified Bushen Jianpi Tiaochong decoction has definite efficacy in treating PCOS complicated with infertility, can increase endometrial thickness, help control sex hormone levels, and ensure safety. It is worthy of application.
