社区老年高血压患者血压控制效果及影响因素分析

Analysis on Blood Pressure Control Effect and Influencing Factors of Elderly Patients with Hypertension in Community

目的:分析社区老年高血压患者血压控制效果及影响因素。方法:选取2019年6月-2022年12月北京市朝阳区亚运村社区卫生服务中心收治的1 260例社区老年高血压患者作为研究对象,根据血压水平分为达标组(n=617)和不达标组(n=643)。比较两组基本情况、治疗依从性、知信行现状,分析社区老年高血压患者血压控制效果的影响因素。结果:血压达标组文化程度小学及以下、体质量指数(BMI)≥25 kg/m^(2)、有并发症、吸烟、高盐饮食、依从性差、知信行现状不合格占比低于不达标组,差异有统计学意义(P<0.05)。文化程度小学及以下、BMI≥25 kg/m^(2)、有并发症、高盐饮食、依从性差、知信行现状不合格是社区老年高血压患者血压控制效果的独立影响因素(P<0.05)。结论:临床应重视文化程度小学及以下、BMI≥25 kg/m^(2)、有并发症、高盐饮食、依从性差、知信行现状不合格社区老年高血压患者的血压控制效果,为其制定干预方案,以提高血压控制效果。

Objective:To analyze the blood pressure control effect and influencing factors of elderly patients with hypertension in the community.Methods:A total of 1260 elderly patients with hypertension in the community who were admitted to Beijing Chaoyang District Asian Games Village Community Health Service Center from June 2019 to December 2022 were selected as the study subjects.According to the blood pressure level,they were divided into the qualified group(n=617)and the unqualified group(n=643).The basic situation,treatment compliance,knowledge-belief-practice status of the two groups were compared,and the influencing factors of blood pressure control effect of elderly patients with hypertension in the community were analyzed.Results:The proportions of education level of primary school or below,body mass index(BMI)≥25 kg/m^(2),complications,smoking,high-salt diet,poor compliance,and unqualified knowledge-belief-practice status in the blood pressure qualified group were lower than those in the unqualified group,and the difference was statistically significant(P<0.05).Education level of primary school or below,BMI≥25 kg/m^(2),complications,high-salt diet,poor compliance,and unqualified knowledge-belief-practice status were the independent influencing factors of blood pressure control effect in elderly patients with hypertension in the community(P<0.05).Conclusion:In clinical setting,attention should be paid to the blood pressure control effect of elderly patients with hypertension in the community who have education level of primary school or below,BMI≥25 kg/m^(2),complications,high-salt diet,poor compliance,and unqualified knowledge-belief-practice status,and an intervention plan should be formulated to improve the blood pressure control effect.
