外来医疗器械及植入物精细化管理在骨科植入物手术感染控制中的应用研究

Application of refined management of external medical devices and implants in infection control of orthopedic implant surgery

目的探讨骨科植入物手术中应用外来医疗器械及植入物精细化管理的效果。方法选取2021年3月—2023年4月在郑州大学第一附属医院接受骨科植入物手术的患者268例,按是否应用精细化管理方法分组,将2021年3月—2022年3月未采用精细化管理的134例作为常规组,将2022年4月—2023年4月采用精细化管理的134例作为试验组,比较两组医疗器械合格率及使用满意度和院内手术切口感染率。结果试验组外来医疗器械合格率为96.02%,高于常规组的83.99%(χ^(2)=26.345,P<0.001);试验组外来医疗器械使用满意度的物品质量、器械配送、供应及时性和问题改进评分为(85.23±6.73)分、(86.34±6.63)分、(85.67±6.83)分和(88.43±6.58)分,均高于常规组(P<0.05);试验组院内手术切口感染率0.75%,低于常规组的2.24%(χ^(2)=6.259,P<0.001)。结论将精细化管理手段应用在骨科植入物手术的感染控制当中,可以提升外来医疗器械及植入物的合格率与使用满意度,同时可以降低院内手术切口感染发生率,值得推广与应用。

Objective To observe and analyze the effect of infection control in orthopedic surgery with the application of external medical instruments and implants refined management.Methods Case data were selected from March 2021 to April 2023,268 patients who received orthopedic implant surgery were selected as the main study cases,and the implementation of fine management method was used as the basis for the division of two management groups,the conventional group and the experimental group.The conventional group was selected from March 2021 to March 2022 and has not received refined management.The experimental group received intensive management from April 2022 to April 2023.The number of patients in both groups was 134.The effects of the two management methods on surgical infection control were analyzed.Results The qualified rate of foreign medical devices in experimental group was 96.02%,which was higher than that in conventional group(83.99%)(χ^(2)=26.345,P<0.001).The scores of item quality,device distribution,timeliness of supply and problem improvement were(85.23±6.73)points,(86.34±6.63)points,(85.67±6.83)points and(88.43±6.58)points in the satisfaction degree of foreign medical devices in experimental group,which were higher than those in conventional group(P<0.05).The infection rate of hospital incision in experimental group was 0.75%,which was lower than that in conventional group(2.24%)(χ^(2)=6.259,P<0.001).Conclusion The application of refined management in the infection control of orthopedic implant surgery can improve the qualified rate and satisfaction of external medical instruments and implants,and reduce the incidence of incision infection in hospital surgery,which is worthy of promotion and application.
