瑞马唑仑对老年腹腔镜手术患者应激反应指标与术后认知功能的影响评估

Evaluation of Effects of Remazolam on Stress Response Indexes and Postoperative Cognitive Function in Elderly Patients Undergoing Laparoscopic Surgery

目的探究瑞马唑仑对老年腹腔镜手术患者应激反应指标与术后认知功能的影响。方法单纯随机选取2020年1月—2023年1月山东省泰安市肿瘤防治院收治的50例老年腹腔镜手术患者作为研究对象,按照随机数表法分为研究组和对照组,每组25例。研究组患者麻醉诱导时静脉注射瑞马唑仑0.15 mg/kg,对照组患者注射等量生理盐水。对比术后两组患者的睫毛反射消失时间、脑电双频指数(Bispectral Idex,BIS)降至目标范围时间、术后认知功能和并发症发生率。结果研究组患者的睫毛反射消失时间(29.26±3.28)s、BIS降至目标范围时间(52.65±2.39)s明显短于对照组的(57.12±1.67)、(79.24±2.58)s,差异有统计学意义(t=37.846、37.803,P均<0.05)。研究组术后认知功能明显优于对照组,差异有统计学意义(t=10.500,P<0.05)。研究组并发症总发生率(8.00%)明显低于对照组(32.00%),差异有统计学意义(χ^(2)=4.500,P<0.05)。结论对于老年腹腔镜手术患者而言,采用瑞马唑仑能够有效的缩短患者的睫毛反射消失时间、BIS降至目标范围时间,降低并发症发生率,提升患者术后认知功能。

Objective To explore the effects of remazolam on stress response indexes and postoperative cognitive function in elderly patients undergoing laparoscopic surgery.Methods Fifty elderly patients with laparoscopic surgery admitted to Tai'an Cancer Prevention and Control Hospital of Shandong Province from January 2020 to January 2023 were simple and randomly selected as the study objects,and they were divided into study group and control group according to the random number table method,with twenty-five cases in each group.In the study group,0.15 mg/kg remazolam was injected intravenously during anesthesia induction,while the same amount of normal saline was injected in the control group.The disappearance time of eyelash reflex,the time for bispectral idex(BIS)to fall to the target range,the postoperative cognitive function and the incidence of complications were compared between the two groups.Results The disappearance time of eyelash reflex(29.26±3.28)s and the time for BIS to target range(52.65±2.39)s in the study group were significantly shorter than those in the control group of(57.12±1.67)s and(79.24±2.58)s,and the differences were statistically significant(t=37.846,37.803,both P<0.05).The postoperative cognitive function of the study group was significantly better than that of the control group,and the difference was statistically significant(t=10.500,P<0.05).The total incidence of complications in the study group(8.00%)was significantly lower than that in the control group(32.00%),and the difference was statistically significant(χ^(2)=4.500,P<0.05).Conclusion For elderly patients undergoing laparoscopic surgery,the use of remazolam can effectively shorten the disappearance time of eyelash reflex and the time for BIS to fall to the target range.It can reduce the incidence of complications,improve postoperative cognitive function of patients.
