子宫下段连续折叠缝合术在瘢痕子宫孕妇剖宫产中的效果及安全性分析

The effect and safety analysis of continuous folding suturing of the lower segment of the uterus in cesarean section of pregnant women with scarred uterus

目的 评估子宫下段连续折叠缝合术在瘢痕子宫孕妇剖宫产中的效果及安全性。方法 选取本院收治的85名患有瘢痕子宫的孕妇为研究对象,依据所采用的不同治疗策略分为两组,对照组42例,研究组43例。对照组实施标准的缝合手术以及缩宫素静脉滴注治疗,观察组在对照组的基础上增加了子宫下段的连续纵向折叠缝合术。比较两组产后出血量、输血量、住院时间、凝血功能和血红蛋白水平方面的差异。结果 观察组产后出血量、输血量低于对照组,住院时间短于对照组(P<0.05)。手术后1 d,观察组的血小板计数、凝血酶时间、凝血酶原时间以及血红蛋白水平均显著优于对照组(P<0.05)。结论 在瘢痕子宫孕妇的剖宫产中应用子宫下段连续折叠缝合术,能有效降低产后出血量,减少输血需求,缩短住院时间,改善术后恢复,并发症风险低,值得推广。

Objective To evaluate the effectiveness and safety of continuous folding suturing of the lower segment of the uterus in cesarean section of pregnant women with scarred uterus.Methods 85 pregnant women with scarred uterus admitted to our hospital were selected as the research subjects.They were divided into two groups based on different treatment strategies:a control group of 42 cases and a study group of 43 cases.The control group received standard suturing surgery and intravenous infusion of oxytocin,while the observation group received continuous longitudinal folding suturing of the lower segment of the uterus in addition to the control group.Compare the differences in postpartum hemorrhage volume,transfusion volume,hospital stay,coagulation function,and hemoglobin levels between two groups.Results The postpartum hemorrhage and transfusion volume in the observation group were lower than those in the control group,and the hospital stay was shorter than that in the control group(P<0.05).After 1 day of surgery,the platelet count,thrombin time,prothrombin time,and hemoglobin levels in the observation group were significantly better than those in the control group(P<0.05).Conclusion Continuous folding and suturing of the lower segment of the uterus can effectively reduce postpartum bleeding,reduce transfusion needs,shorten hospital stay,improve postoperative recovery,and have a low risk of complications in cesarean section of pregnant women with scarred uterus.It is worth promoting.
