连续腰硬联合阻滞麻醉在无痛分娩中的镇痛效果及对产程和母婴状况的影响研究

Study on the analgesic effect of continuous lumbo-epidural block anesthesia in painless delivery and its influence on labor and maternal and infant conditions

目的探究连续腰硬联合阻滞麻醉在无痛分娩中的镇痛效果及对产程和母婴状况的影响。方法选取50例产妇,按照随机数字表法分组,每组25例。对照组产妇使用自然分娩,研究组产妇使用连续腰硬联合阻滞麻醉无痛分娩,比较两组产妇产程时间疼痛情况以及新生儿Apgar评分。结果宫口开3cm后、宫口开全、胎儿娩出时,研究组产妇疼痛评分较对照组明显下降(P<0.05)。研究组第一、第二产程时间均短于对照组(P<0.05),两组第三产程时间比较,差异无统计学意义(P>0.05)。研究组新生儿Apgar评分为(9.44±0.14)分,对照组新生儿Apgar评分为(9.38±0.22)分,比较差异无统计学意义(P>0.05)。结论连续腰硬联合阻滞麻醉在无痛分娩中的镇痛效果显著,可以改善产妇的分娩体验,加快产程进展,且对母婴结局无不利影响,值得临床推广。

Objective To explore the analgesic effect of continuous epidural block anesthesia in painless labor and its influence on the labor process and maternal and infant conditions.Methods 50 parturients were selected and divided into 25 cases in each group according to random number table method.The women in the control group were given natural delivery,and the women in the study group were given continuous lumbo-epidural block anesthesia and painless delivery.The pain during labor and neonatal Apgar score were compared between the two groups.Results Compared with the control group,the pain scores of women in the study group decreased significantly after 3 cm of uterine opening,full uterine opening and fetal delivery(P<0.05).The time of the first and second stage of labor in the study group was shorter than that in the control group(P<0.05),but there was no significant difference in the time of the third stage of labor between the two groups(P>0.05).The Apgar score of neonates in the study group was(9.44±0.14)points and that in the control group was(9.38±0.22)points,with no significant difference(P>0.05).Conclusion lumbar and epidural block anesthesia has significant analgesic effect in painless labor,which can improve maternal labor experience and accelerate labor progress,and has no adverse effect on maternal and infant outcomes,which is worth promoting.
