阴道微环境变化与高危型HPV感染及宫颈病变的相关性研究

摘要 目的探讨阴道微环境与高危型人乳头瘤病毒(HR-HPV)感染及宫颈病变的相关性。方法选取120例宫颈病变患者作为观察组;另选取同期120例进行健康体检的健康者作为对照组。对受试者进行阴道微生态平衡、阴道菌群及HR-HPV感染检测。对比分析阴道微生态失调、阴道菌群检出率、HPV阳性率及HPV分型的构成,并分析其相关性。结果与对照组相比,观察组阴道微生态失调率、HPV阳性率、乳酸杆菌、线索细胞、阴道滴虫检出率更高(P<0.05);阴道微生态失调、HPV阳性均与宫颈疾病成正相关(r=0.675、0.698,P=0.020、0.016)。结论宫颈病变与阴道微生态失衡、HPV感染密切相关,临床应加强对宫颈疾病患者阴道微生态及HPV感染状态的研究,为临床制订合理的诊疗策略提供理论依据。 Objective To look into the connection between cervical lesions,high-risk human papillomavirus(HR-HPV)infection,and the vaginal microenvironment.Methods 120 patients with cervical lesions were selected as the observation group.Another 120 healthy subjects who underwent physical examination during the same period were selected as the control group.The subjects were tested for vaginal microecological balance,vaginal flora and HR-HPV infection.The vaginal microecological disorder,the detection rate of vaginal flora,the positive rate of HPV and the composition of HPV types were compared and analyzed.Results Compared with the control group,the observation group had higher rates of vaginal microbiota dysbiosis,HPV positivity,detection of lactobacilli,clue cells,and vaginal trichomonas(P<0.05);Vaginal microecological disorder and HPV positive were positively correlated with cervical disease(r-0.675、0.698,P=0.020、0.016).ConclusionCervical lesions are closely related to vaginal microecological imbalance and HPV infection.Therefore,strengthening the study of vaginal microecological status and HPV infection status in patients with cervical diseases will provide theoretical basis for clinical formulation of reasonable diagnosis and treatment strategies.
