宫颈球囊联合缩宫素对引产凝血指标、阴道分娩率、促宫颈成熟效果的影响

Effect of Cervical Balloon Combined with Oxytocin on Coagulation Index,Vaginal Delivery Rate and Cervical Ripening Effect of Induced Labor

目的 研究宫颈球囊联合缩宫素对引产产妇凝血指标、阴道分娩率、促宫颈成熟效果的影响。方法 选取2021年6月-2023年5月天津港口医院具备引产指征的200例孕妇为研究对象,采用随机数字表法分为对照组(100例)与观察组(100例)。对照组采用缩宫素引产方案,观察组应用宫颈球囊联合缩宫素引产方案,比较两组引产时间、阴道分娩率、凝血指标[纤维蛋白原(Fib)、二聚体(D-D)]、促宫颈成熟效果(Bishop宫颈成熟度评分)、不良结局。结果 观察组引产时间短于对照组,且阴道分娩率高于对照组(P<0.05)。两组引产后Fib、D-D低于引产前,且观察组Fib、D-D低于对照组(P<0.05);用药10 h后,两组Bishop宫颈成熟度评分高于用药前,且观察组Bishop宫颈成熟度评分高于对照组(P<0.05);观察组不良妊娠结局发生率低于对照组(P<0.05)。结论 宫颈球囊联合缩宫素可缩短产妇引产时间,提高其阴道分娩率,改善凝血指标,加速宫颈成熟,降低不良妊娠结局的发生风险。

Objective To study the effect of cervical balloon combined with oxytocin on coagulation index,vaginal delivery rate and cervical ripening effect of induced labor.Methods A total of 200 pregnant women with indications for induced labor in Tianjin Port Hospital from June 2021 to May 2023 were selected as the research objects and divided into control group(100 pregnant women)and observation group(100 pregnant women)by random number table method.The control group was treated with oxytocin induction program,and the observation group was treated with cervical balloon combined with oxytocin induction program.The induction time,vaginal delivery rate,coagulation index[fibrinogen(Fib),dimer(D-D)],cervical ripening effect(Bishop cervical maturity score),and adverse outcomes were compared between the two groups.Results The induction time of the observation group was shorter than that of the control group,and the vaginal delivery rate was higher than that of the control group(P<0.05).After induction of labor,Fib and D-D in the two groups were lower than those before induction of labor,and Fib and D-D in the observation group were lower than those in the control group(P<0.05).After 10 hours of medication,the Bishop cervical maturity scores of the two groups were higher than those before medication,and the Bishop cervical maturity score of the observation group was higher than that of the control group(P<0.05).The incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group(P<0.05).Conclusion Cervical balloon combined with oxytocin can shorten the time of induced labor,improve the rate of vaginal delivery,improve the coagulation index,accelerate cervical ripening,and reduce the risk of adverse pregnancy outcomes.
